Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-16-Q-0887 is issued as a Request for Quotation (RFQ). This requirement is set aside for SDVOSB and VOSB. NAICS: The NAICS is 334510 and the small business size standard is 1250. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. To be receiving a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to received a contract award. Offeror may register with CCR at www.sam.gov. Period of Performance (POP): The base POP is from 10/01/2016 to 09/30/2017. Description of Services: Carestream Detector Parts only agreement with supplemental detector accidental damage replacement plan 1.SCOPE: Contractor shall provide parts only coverage for the equipment listed in attachment A. The contractor shall also provide supplement coverage for detector head impact/drop/collision resulting in damage and subsequent replacement. There is no onsite support with this agreement. 2.CONTRACT PERIOD: Contract period shall be 10/1/2016 to 9/30/2017. 3.QUALIFICATIONS: Bidders must provide, upon request, documentation of factory certified service/maintenance training on the specific equipment under the terms of this contract. The Contracting Officer and/or Contracting Officer's Representative (COR) specifically reserves the right to reject any of the Contractor's personnel and refuse them permission to work on the equipment outlined in Attachment 1 (see below), based upon credentials provided. The contractor shall maintain sufficient stock of parts needed for repair and maintenance of the equipment, or be able to secure parts within three (3) business days. All bidders must warrant that any parts it supplies will be free from defects in material and workmanship during the service period 4.SERVICES TO BE PROVIDED: A.General: 1)Parts only agreement. The contractor will provide parts upon request once troubleshooting has been completed. 2)If detector is damaged as a result of being dropped, impacted, infiltrated by fluid, broken or cracked glass, regardless of the cause, the detector will be replaced once the deductible is met, as set in the agreement. B.Repair: 1)No repair services are to be provided with this requirement C.Preventive Maintenance (PM): 1)No Preventative Maintenance will be provided with this requirement D.Documentation: Documentation will provided for all parts replaced for tracking purposes 5.TERMS AND CONDITIONS: A.User Responsibility: The contractor will not be held responsible under the terms of this contract for repairs to equipment necessitated by user abuse and/or misuse of equipment.
