Description

JOFOC (Justification for Other than Full and Open Competition) Notice of Award The Great Plains Area Office intends to award a sole-source, Buy-Indian award with the Standing Rock Sioux Tribe for the Standing Rock Tribal Diabetes Program to provide high quality Diabetes education, annual screenings, continuity of care, identification of patient's needs, and further treatment of care for the Ft. Yates IHS Hospital in Ft. Yates, ND. A Special Diabetes Program for Indians (SDPI) Community-Directed Grant Program (CD16) Award was derived per FOA HHS-2016-IHS-SDPI-0001 by the Standing Rock Service Unit with an approved Notice of Award designating a sub-contractor/sub-grantee. The resultant contract award shall consist of a Base Year and Four (4) One Year Options: September 14, 2016 to December 31, 2016 January 1, 2017 to December 31, 2017 January 1, 2018 to December 31, 2018 January 1, 2019 to December 31, 2019 January 1, 2020 to December 31, 2020 Attached is a Justification for Other Than Full and Open Competition (Federal Acquisition Regulation 6.302-1 & 5 (25 USC 47) and the Notice of Agreement.
