- 1.This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the FAR (Federal Acquisition Regulation) Part 13 and Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Combined Synopsis/Solicitation requested and a written solicitation will not be issued. 2.The Combined Synopsis/Solicitation Number is: VA258-16-Q-0933. This Combined Synopsis/Solicitation is issued as a Request for Quotation (RFQ). The award will be a FIRM FIXED PRICE CONTRACT. 3.Quotes are due via email to the Contracting Specialist, Thomas W. McKendry at thomas.mckendry@va.gov with VA258-16-Q-0933 Epoxy Flooring in the SUBJECT line by 1500 MST 21 September 2016. NO EXCEPTIONS. Duns Number and Cage code are required with quote submission. 4.Question Submission: Interested offerors may submit any questions concerning the solicitation prior to __1500 MST 20 September 2016__. Questions must be submitted by email to Thomas W. McKendry at thomas.mckendry@va.gov with VA258-16-Q-0933 Epoxy Flooring in the subject line. Questions not received within the time or format specified will not be considered. No telephonic requests will be entertained. All responses to questions will be emailed to all interested parties. All questions must be addressed to the Contracting Specialist only. Any other official may disqualify the offeror from submission of a quote. 5.A site visit will be held at 9:30am MST 20 September 2016 at VA Cooperative Studies Program, Clinical Research Pharmacy Coordinating Center, 2401 Centre Avenue, SE, Albuquerque, NM 87106-4180. The Government point of contact will be David. Older. No later than 12:00pm the day before the site visit, the names of individuals attending must be emailed to david.older@va.gov. The delivery location is the: VA Cooperative Studies Program Clinical Research Pharmacy Coordinating Center 2401 Centre Avenue, SE Albuquerque, NM 87106-4180 Statement of Work (SOW) VA258-16-Q-0933 Epoxy Flooring for Mezzanine 1. TITLE Installation of epoxy flooring 2. BACKGROUND The Clinical Research Pharmacy Coordinating Center (CRPCC) is expanding its biorepository function. The current mezzanine flooring is plywood sheeting prone to damage so it needs installation/application of epoxy flooring material. Pallets of packaging materials and tissue storage freezers are/will be located on the surface. 3. SCOPE The CRPCC has a mezzanine located in the warehouse section ("Annex") of the facility with some pharmaceutical manufacturing related equipment and functions underneath it. The mezzanine currently has an exposed plywood floor. To eliminate damage to the surface, it needs to be covered with epoxy flooring material and suitable substrate to prevent flexing/cracking of the epoxy. The area to be covered is approximately 2950sf. The Vendor is responsible for all measurements. The area is bordered by a wall down one side which requires the epoxy to be applied to create a cove base along the wall. The remaining sides will be flat and the epoxy shall transition to a vinyl floor approximately 8ft wide where they meet. 4. SPECIFIC TASKS The following section specifies the Performance Objectives and Performance Elements for the contract. 4.1 Install appropriate substrate material and epoxy coating to cover 2950 sq ft. 4.1.1 Floor coating shall meet or exceed: ACI 503R Adhesion standard of 300 psi (concrete failure). 4.1.2 Floor coating shall meet or exceed: ASTM D 412 Tensile Strength of 1,200 psi. 4.1.3 Floor coating shall meet or exceed: Hardness, Shore D ASTM D 2240 23. 4.1.4 Floor coating Shall meet or exceed ASTM C 884 Thermal Cycling (24Hours, -21C to 25c) by not cracking. 4.1.5 Floor coating shall meet or exceed ASTM D 412 Elongation at Break - 145% 4.1.6 Material shall be color matched to Armstrong Medical Grade vinyl flooring (Code: 88202 PEBBLEWASH) 4.1.7 Vendors shall provide proof that their product submission meets or exceeds the specifications listed in this section by providing literature in significant detail and quantity, showing exactly how they meet or exceed the specifications provided here 4.1.8 Vendors must demonstrate previous experience installing epoxy flooring on wooden substructure by providing references for three similar installations. Referenced experience must be of similar size and scope. The Vendor must provide, at a minimum, a name, phone number, location of the flooring, square footage and epoxy product used. 4.2 Provide flooring materials (substrate materials and epoxy). 4.3 Install materials. Upon reaching the facility access gate, push the call button for Security ¦even if gate is open, visitors must push call button for Security to allow access When Vendors enter the front door of the facility, Security will open door, and then visitors will exchange a photo ID for a Visitor Badge and MUST BE escorted while in the facility. When on facility grounds, Vendors will be escorted by a CSPCRPCC employee at all times as mandated and strictly enforced in facility security policy. 8. GOVERNMENT-FURNISHED EQUIPMENT (GFE)/GOVERNMENT-FURNISHED INFORMATION (GFI) GFE: The COR will coordinate assistance for CRPCC employees to provide forklift assistance and other support as requested and within CRPCC capabilities to the contractor to get materials to the installation location. GFI: The COR will either directly provide or will coordinate contact with applicable party to answer any vendor technical questions regarding this project. 9.
