- W900KK-16-T-0027 RFQ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Please refer to the attached Request for Quote (RFQ), Standard Form (SF) 1449 #W900KK-16-T-0027, for the Statement of Work (SOW), evaluation criteria, list of contract line item number(s), set-aside information, NAICS code and small business size standard, and other details. Your quote is due by 12:00 P.M. EST on Monday, September 19, 2016. Please send your quote via email to the Contract Specialist, Jan A. Forman, at jan.a.forman.civ@mail.mil referencing the RFQ# W900KK-16-T-0027 in the subject line. Attachments in Microsoft Office or pdf files are acceptable. Late submissions will not be accepted. GENERAL REQUIRMENTS: The US Army Project Manager (PM) Medical Simulation (MedSim) has a requirement to deliver various healthcare Training Supplies, to include, but not limited to, Basic Life Support (BLS), Advance Cardiac Life Support (ACLS), Heartsaver® First Aid, and other, to the Veterans Health Administration (VHA) Resuscitation Education Initiative (REdI) Simulation Learning, Education and Research Network (SimLEARN) program. The attached SOW defines the requirement for the Training Supplies for the Veterans Health Administration (VHA) Simulation, Education, and Learning Network (SimLEARN). OTHER REQUIREMENTS All items to be delivered under this contract shall be packaged, packed and marked to prevent deterioration and damage during shipping, handling and storage to ensure safe arrival at destination. The Training Supplies shall be delivered to the FOB Destination point under Delivery Information, specified in the Requirements section of the SOW. All inspection and acceptance shall be at destination by the Government. Final inspection to be performed at the delivery site will be directed and witnessed by the technical representative of the Procuring Contracting Officer (PCO). Final acceptance of the Contract Line Item(s) will be evidenced by signature of the PCO or his/her duly authorized representative. Please apply quantity discounts as applicable. This is a Request for Quote only. Do not proceed with this work until you have received a contract document signed by the PCO.
