NOTICE TYPE: Request for Information POST DATE: September 9, 2016 RESPONSE NLT DATE: September 23, 2016 @ 2:00 PM EST NAICS CODE: 339113 - Surgical Appliance and Supplies Manufacturing SYNOPSIS THIS IS NOT A SOLICITATION: This is a Request for Information notice issued in accordance with FAR 10.002(b) to conduct market research. This Request for Information is issued solely for information and planning purposes - it does not constitute a Request for Proposal (RFP) - or a promise to issue a RFP in the future. This request for information does not commit the Government to contract for any supply or services whatsoever. The Department of Veterans Affairs (VA) is not, at this time, seeking proposals and will not accept unsolicited costs incurred in response to this Request for Information; all costs associated with responding to this Request for Information will be solely at the interested vendor's expense. Not responding to this Request for Information does not preclude participation in any future RFP, if any is issued. Any information submitted by respondents to this Request for Information is strictly voluntary. All submissions become Government property and will NOT be returned. CONTRACTING OFFICE ADDRESS: U.S. Department of Veterans Affairs Strategic Acquisition Center 10300 Spotsylvania Ave Fredericksburg, VA 22408 POINT OF CONTACT: Priscilla Burke Contract Specialist priscilla.burke@va.gov or Melissa Lera Contracting Specialist melissa.lera@va.gov OVERVIEW The Department of Veterans Affairs, Veterans Health Administration (VHA) is requesting information for ENFit Feeding Tubes, Syringes, and Bags. Veterans Health Administration (VHA) has a need for a full complement of feeding appliances (enteral syringes, enteral tubing, and bags). VHA is aware of a need for a streamlined feeding formulary but, are unable to identify a way to accomplish and standardize it. VHA is seeking feedback from the vendor community as to its capability of supplying the feeding appliances. The goal of VHA in feeding care is to provide the VHA nurses and patients with feeding tubes, syringes, bags and administration sets that prevent wrongful connections between tube feedings and intravenous lines. These appliances would be used for routine, long term care by patients. This RFI may result in a solicitation. This RFI is intended to assess industry's capability to support and/or develop an adequate solution to the Government's continuing need(s). The Strategic Acquisition Center (SAC) is requesting responses from qualified business concerns; the qualifying NAICS code for this effort is 339113 (Surgical Appliance and Supplies Manufacturing). INFORMATION REQUESTED FROM INDUSTRY In response to the Request for Information, interested Contractor's shall submit the following information: 1. FEEDING APPLIANCES AND SUPPLIES Based upon the historical purchase data as well as your product knowledge, please develop a mock formulary of products your company recommends for VA facilities. Please limit this mock formulary to no more than 100 items. Please explain why each product is a 'must have' and the benefits of each. Please indicate whether each product is ENfit compatible. Please indicate whether each item is available via Department of Veterans Affairs Federal Supply Schedule (VA FSS), and whether your company would make these items available via the VA Medical Surgical Prime Vendor Program (MSPV). If the product is on VA FSS, please indicate the VA FSS Contract Number. You may reference data in your product literature if you include it. 2. COMPANY INFORMATION/SOCIO-ECONOMIC STATUS: a.Provide the company size, DUNS #, Cage Code, and POC information (name, e-mail, address, telephone, and fax numbers. b.VA has identified the appropriate North American Industry Classification System (NAICS) Code 339113 - Surgical Appliance and Supplies Manufacturing which has a size standard of 500 Employees for this Request for Information. Please identify and explain any other NAICS codes your company believes would better represent the listed equipment. 1.Indicate whether your company, Sub-Contractors, teaming partners, or joint ventures has a Federal Socio-Economic Status, e.g., Service-Disabled Veteran Owned Small Business, Veteran Owned Small business, Women-Owned Small Business, Disadvantaged Small business, Hub Zone Business, Small Business. If Service Disabled or Veteran Owned Small Business, is your company and/or partners registered and verified in VA's Vet-Biz repository? 2.Indicate whether your company is the manufacturer or authorized distributor. 3.Please provide your company's responses in the following format: Product descriptionProduct number Cost Example: BAG,TUBE FEEDING,1200ML CONSOLIDATED #2049Product code Next product Next product SPECIFIC RESPONSE INSTRUCTIONS Submit your response in accordance with the following: a.Include the name, e-mail address, and phone number of the legally and contractually binding representative of your company b.Submit any questions NLT September 23, 2016 @ 2:00 PM EST c.Submit your response/questions/requests via e-mail to priscilla.burke@va.gov with a copy to melissa.lera@va.gov d.Mark your response as "Proprietary Information" if the information is considered business sensitive e.Submit your response and place Attention: Feeding Appliances and RFI# VA119-16-N-0656 in the subject line of your response email. f.Please provide clinical information on your products to include a streamlined, "mock" formulary of products that would result in positive clinical outcomes when used for the majority of our patients. g.Please present your findings utilizing the Pareto Principle or '80/20 rule' i.e. please indicate which 20% of products will work for 80% of patients. 3. BACKGROUND/PAST EXPERIENCE: Provide the following information on three (3) contracts for a healthcare organization of similar size or scope completed within the last three (3) years for which the responder was a prime or Sub-Contractor. The name, address, and value of each contract; a.The name, telephone and email address of the Contracting Officer and Contracting Officer's Technical Representative of each project; b.A description of each contract, including difficulties and successes; and c.Your company's role provided for each contract. 4. PRICING: Provide your commercial and/or published catalog or list price history for the same or similar products/solutions.
