- Cover Letter and Document Checklist Form Form Form SF-1149 with complete requirements. this is a combined synopsis/solicitation for commericl item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will be issued. The request for Proposal (RFP) solicitation number is RFP: 245-M16-0016. The solicitation document and incorporated provisions and clauses are those in efect through Federal Acquisition Circular FAC 2005-89 effective August 15, 2016. This requirement is solicited as Full & Open competition. The associated NAICS code is 621999 and the small business size standard is $7million. See Section B-2, pages. no. 3-4, section titled - Fee Schedule/Pricing of the solicitation for the fee schedule proposed for this requirement. Description: The Contractor shall provide all medically necessary health care services to Indian Health Service (IHS) Purchased/Referred Care (PRC) eligible patients on a non-discriminatory basis. In providing health care services, the Contractor assumes the same responsibilities and provides the same consideration, advise, instrction, personal home care uidance and quality of treatment of IHS patients under the terms of this contract s is provided to the general public. the IHS has instituted a pyment policy which was published in the Federal Register to contract for health care service under Purchased/Referred Care (PRC) program for eligible American Indians and Alaskan Natives only with those health care providers who agree to accept, as payment in full, reimbursement at rates no higher than the prevailing Medicare Reimbursement Methodoligies including deductibe and coinsurance for allowable charges. It is the intent to establish Multiple Award Indefinite-Quantity type contracts with those providers who will accept Medica allowing rates as reimbursement for authorized health care services rendered. The provision at 52.212-1: Instructions to Offerors-Commercial Items, applies to this acquisition and a statement reqarding any addenda to the provision. The provision at 52.212-2: Evaluation-Commercial Items, applies to this solicitation, and the Government will award a contract resulting from the solicitation to the responsible offeror whose ofer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offerors: 1-The Contractor's facility shall be The Joint Commission accreditied or certified by the Centers for Medicard and Medicaid (CMS) for the term of the contract. 2-Professional staff, directly hired by the facility or contracted with the facility shall be licensed in the state in which th facility is located and board eligible/certified by the appropriate medical/professional board/s; 3-The Contractor shall possess Medical Insurance Liability in accordance with FAR 52-237-7, Indemnification and Medical Liability Insurance. In addition, the Contractor shall provide evidence of general liability insurance for all employees of the facility that provide direct patient care, including x-ray, laboratory and pharmacy; and all employees of the facility that provide direct patient care, including x-ray, laboratory and pharmacy; and 4-The Contractor shall have the ability to bill Alternate Resources in accordance with 42.CFR 136a. Offerors shall include a completed copy of the provision at 52.212-3: Offeror, Representations and Certifications-Commercial Items, with it's offer. Clause at AR 52.212-5: Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. Additional contract requirement/s or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices are located in Section C: Contract Clauses, of this solicitation. This solicitation is issued on an "Open Continuous" basis therefore, responses will be considered until the official closing date of August 31, 2017.
