Description

Instructions to Offerors Statement of Work This is a COMBINED SOLICITATION/SYNOPSIS for commercial items/service prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to award a Firm Fixed Price (FFP) contract, utilizing procedures under FAR part 13. The proposed contract action is to provide an automated clinical chemistry analyzer for patient laboratory testing at the Uintah & Ouray (U&O) Service Unit Laboratory (Fort Duchesne, Utah). This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number IHS1285027. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-88, 16 May 2016. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The North American Industry Classification System (NAICS) Code for this acquisition is 334516; Size Standard is 1,000 employees. ALL ITEMS ARE DESIGNATED AS BRAND NAME OR EQUAL. MANUFACTURER, MODEL NUMBERS AND SALIENT SPECIFICATIONS ARE LISTED FOR REFERENCE ONLY; PRODUCTS MEETING OR EXCEEDING THE LISTED SPECIFICATIONS WILL BE CONSIDERED AS EQUAL. ALTERNATE PRODUCTS PROPOSED SHALL BE ACCOMPANIED BY A MANUFACTURERS PRODUCT SPECIFICATION SHEET. Please see attached documents for requirement details.
