Description
The purpose of this amendment is to address the items below and answer vendor questions. CHANGES: 1.Page 10 B(12) & Page32: Remove references to statement of work and replace with "Technical requirements." 2.Remove any reference to CellaVision and replace with "ability to view cells on a monitor". 3.Page 11 B(k): Further define removal of equipment by contractor to equipment belonging to contractor. 4.Page 11 D(3) & (4): Add definitions for consumables and disposables. 5.Page 12 E: Further clarify service hours. 6.Page 12 E (1) f and i: Remove statement "All repairs will be invoiced to customer on the agreed to price list" and replace with "CPRR agreements include all service repairs at no additional charge." 7.Page 12 E 1(h): Reword statement and add "according to manufacturer's recommendations." 8.Page 13 F 1(d): Redefine downtime summary report and add items to include in report. 9.Page 14 J(4): Request for an installation timeline. 10.Page 15 L(1): Redefine instrumentation footprint and add locations that have carts. 11.Remove any reference to "slidestainer" and replace with "Slidemaker/Stainer". 12.Page 32 Part 1: state where the technical requirement is found. 13.Page 32 Part 1: Add clarification to company literature question. 14.Changed the date for closing from 08/25/2016 4pm PST to 09/01/2016 4pm PST. 15.Change the date/time in block 8 from 08/25/2016 4pm PST to 09/01/2016 4pm PST. 16.Nothing else follows. VENDOR QUESTIONS: 1.Page 14. Dimensions of Equipment 40w x 24d. Most vendors offer carts/wagons that the analyzers fit on. 40w X 24d is the space available in most locations. Some locations only have counters that are no larger than 26 inches deep. Carts are welcomed. These locations currently have theirs on a cart but others may like the option. Portland Spokane Boise Walla Walla Roseburg Eugene 2.VA Portland test Volumes appear to be lower than the previous solicitation. Please Confirm. Test Volumes are only an estimate. 3.What sites require a CellaVision? Remove the term "CellaVision" and replace it with 'Ability to view cells on a monitor'. 4.CLIN 0003 - What is to be covered under this CLIN? CellaVision? Is not for CellaVision, it is for all other hematology analyzers and reagents. 5.CLIN 0004 & 0005 - Which sites will use the SMS? Seattle, Spokane, Boise, Alaska 6.CLIN 0006 - What is to be covered under this CLIN? If volumes increase or instrument difficulties require the use of more reagents then additional reagents may be needed. 7.CLIN 0007 What is to be covered under this CLIN? 24/7 service and what accessories? (see question 10 below) Printer, tubes, probes, needles, clamps, grippers, an uninterruptable power supply (USP), racks, etc ¦ Any item needed to run the equipment. Please be specific about what your equipment needs to run and any items provided for it. 8.Page 5. B.2.ii The terms Consumables and Disposables are used differently by different customers. For clarification, please define your understanding of consumables vs. Disposables to be included in the offer. Consumables are considered as anything specific to the analyzers such as (slides, standards, control materials). Disposables are not specific to the instruments. (Test Tubes, pipettes, etc ¦) 9.Page 11. D.8 Please clarify the requirement. "Vendor will supply all Changes in maintenance schedules that affect reagent or cleaning reagent volumes will be changed by the Vendor in accordance with manufacturer's recommendations." Vendor shall supply all reagents or cleaning supplies necessary for manufacturer required changes to the maintenance schedule. All changes must be reported to the laboratory points of contacts. 10.Page 12. E.e requires 24/7 on site response Monday - Friday however CLIN 0007 asks for 24/7. Please clarify. Telephone support 24/7 Onsite service Available M-F 8-430 Local time. Response time for initial calls within 2 hours. 11.E.f requires "All repairs will be invoiced to customer on the agreed to price list." CPRR agreements include all service repairs. Please clarify. Remove E.F. and add the below statement CPRR agreements include all service repairs at no additional charge. 12.F.1.c. Please clarify requirement. Are you looking for MTBF information? Please provide a monthly report of how often and how long your machines are down historically. Average # of downtimes monthly Average hours of downtime monthly Average # of downtime calls from customers per year. # of recalls for reagents or parts in a year. 13.Page 26. E.2.b.4 States that vendors may include product literature, however on Page 30 part II it says that vendors should not include literature. To clarify, the literature may be provided but will not be considered in the evaluation process as part of vendor response. 14. Page 32. The requirement states that the page limit on the technical response is limited to 15 pages and should be tailored specifically to the technical evaluation factors. However in this modified solicitation, the specific sub-factors were removed and replaced with general factors that do not list specifically the technical requirements as they previously did. Please clarify. Please refer to the scope of work starting on page 10 through page 14. This details all of the information needed to submit the quote. The evaluation factors were modified but the requirements remain the same. 15. Slide Stainer is mentioned. We assume you are looking for a Slidemaker/Slidestainer. Is this correct? Yes 16.It states SMS pricing over and under 100 test per day. Can we bed a discount off the AVA CPRR tables? Yes 17.Are you asking for the ability to select service plan by site? No 18.(Page 12, Section E) Is your intent to dictate the PM frequency of no less than every 6 months, regardless of the manufacturer's specification? No please follow manufacture specs. 19.(Page 14, Section K2) We suggest adding that Adjustments to the CPRR payment may also be required where volumes fluctuate over 10% of current plan. Is this possible? Yes 20.(Page 35 Factor 3) For Past Performance, is there a specific number of references you would like? Is there a specific form you want all vendors to use? 3 Past performance references, standard form is attached.
