Description

This is a sources sought notice only, do not submit a quote This service agreement is for listed below. The contractor shall provide unlimited emergency repair and call-back repair services as well as preventative maintenance inspections (2 PM inspections per piece of equipment) to include all parts, labor, travel and all other materials including x-ray tubes for the equipment listed below. This agreement will last through FY17 (10/1/16 - 9/30/17). Equipment - The contractor shall provide maintenance, as described above, on the equipment listed below: EE#Serial #ManufacturerModel 37175K5484-3366Carestream HealthDirectview Max CR Scheduled Maintenance - The contractor will maintain, check and calibrate, according to manufacturer's procedures and recommended schedule for all the included equipment identified above. There will be two (2) preventative maintenance (PM) visits during the contract period. The DirectView Max CR will be scheduled in February and August of each contract year. Submit a one page capability statement by email to andrew.taylor3@va.gov with name, phone number, brief statement on how your company can meet the repair requirements and website information by August 29, 2016 at 4:00 p.m.
