- This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Please advise if you carry this product are a suitable "or equal". EMAIL INFORMATION TO : derrick.fletcher@va.gov STATEMENT OF WORK (SOW) 1.Contract Title. Arxium Service Agreement 2.Background. The Pharmacy department at the Northern Arizona VA Health Care System uses Automed robots to efficiently and accurately dispense medication for patients. The maintenance and uptime of these robots is critical to providing patient centered care by preventing medication mistakes. 3.Scope. Description of Supplies/Services Required to Meet the Agency's Needs: a.Period of Performance: 10/1/2016-9/30/2017 b.Need coverage for the Automed FastFill 220, FastPak EXP, Quickfill, LPUs, and Licenses to include: i.All parts ii.Phone support 24 hours a day, 7 days a week iii.On-site support Monday-Friday, 8am-5pm LST iv.Semi-annual preventive maintenance service 4.Specific Tasks. Task 5.1.The Contractor will notify either Biomedical Engineering or the Pharmacy department of his/her intent to come on-site before arrival. Task 5.2.The Contractor will check-in with the VA Police Department upon arrival, before doing any work on-station. Task 5.3.Work on the Automed FastFill 220 (s/n: 38.E1103) within coverage hours. Task 5.9.a.The Contractor shall offer phone support 24 hours a day, 7 days a week. Task 5.9.b.The Contractor shall offer on-site support Monday-Friday 8am-5pm LST, excluding federal holidays. Task 5.9.c.If the Contractor cannot respond within coverage hours, then he/she will respond as soon as coverage hours have restarted. Task 5.9.d.Preventive Maintenance shall be performed on a semi-annual basis. Task 5.4.Work performed on the Automed Fastpak EXP (s/n: 37.70305963 and 37A.70305963) within coverage hours. Task 5.4.a.The Contractor shall offer phone support 24 hours a day, 7 days a week. Task 5.4.b.The Contractor shall offer on-site support Monday-Friday 8am-5pm LST, excluding federal holidays. Task 5.4.c.If the Contractor cannot respond within coverage hours, then he/she will respond as soon as coverage hours have restarted. Task 5.4.d.Preventive Maintenance shall be performed on a semi-annual basis. Task 5.5.Work performed on the Automed Additional LPU(s/n: 37A.09130057) within coverage hours. Task 5.5.a.The Contractor shall offer phone support 24 hours a day, 7 days a week. Task 5.5.b.The Contractor shall offer on-site support Monday-Friday 8am-5pm LST, excluding federal holidays. Task 5.5.c.If the Contractor cannot respond within coverage hours, then he/she will respond as soon as coverage hours have restarted. Task 5.6.The Contractor shall provide and maintain licenses for (7) workstations. Task 5.7.The Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and confirms quality assurance tests results. Task 5.8.The Contractor shall not remove any hard drives off station. All hard drives will be turned in to Biomedical Engineering. Task 5.9.Records Management Language for Contracts. The following standard items relate to records generated in executing the contract: Task 5.9.a.Citations to pertinent laws, codes, and regulations such as 44 U.S.C. chapters 21, 29, 31, and 33; Freedom of Information Act 5 U.S.C. 552; Privacy Act 5. U.S.C. 552a; 36 CFR Part 1222 and Part 1228. Task 5.9.b. The Contractor shall treat all deliverables under the contract as property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Task 5.9.c.The Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Task 5.9.d.The Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Task 5.9.e.The Contractor shall not create or maintain any records containing any government Agency records that are not specifically tied to or authorized by the contract. Task 5.9.f.The Government Agency owns the rights to all data/records produced as part of this contract. Task 5.9.g.The Government Agency owns the rights to all electronic information and all supporting documentation created as part of this contract. The Contractor must deliver sufficient technical documentation with all data deliverables to permit the Agency to use the data. Task 5.9.h.The Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format, mode of transmission, or state of completion. Task 5.9.i.No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage, or alienation of Federal records is subject to the files and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Task 5.9.j.The Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship in support of this contract requiring the disclosure of information, documentary material, and/or records generated under or relating to this contract. The contractor is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. 5.Performance Monitoring. The Imaging Department and Biomedical Engineering will oversee work done by the Contractor and accept the quality of work as appropriate. 6.Security Requirements. The Contractor will require an annual pass to NAVAHCS, which is distributed by the Police Department on station. The Contractor will have physical and remote service/administrative access to the equipment. This may allow the Contractor to view patient information. 7.Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). The uptime of the equipment is dependent on the facility's utilities, which are provided by the facility. Physical access to the equipment is also dependent on the facility. 8.Other Pertinent Information or Special Considerations. None. a. Identification of Possible Follow-on Work. None. b. Identification of Potential Conflicts of Interest (COI). None. c. Identification of Non-Disclosure Requirements. The Contractor will have physical and administrative access to the equipment. This may allow the Contractor to view patient information. d. Packaging, Packing and Shipping Instructions. All parcels and packages shall come through the NAVHACS warehouse to be distributed either through Biomedical Engineering or the Contractor him/herself if he/she is on station to receive the package. All parcels and packages will ship overnight. e. Inspection and Acceptance Criteria. An Attachment G form will be filled out by the COR upon the expiration of the contract to confirm to what degree the service was satisfactory. 9.Risk Control. There will be no patients scheduled to be in the room with the Contractor. The room housing the equipment will be kept clean in accordance to the standards of the Environmental Management Service line. 10.Place of Performance. Northern Arizona VA Health Care System. Building 107, Room 144 (Brightview) and Room 145 (CardioMD III). 11.Period of Performance. Base year : 10/1/2016-9/30/2017. 12.Delivery Schedule.
