- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. HSHQDC-16-R-01000 is a request for proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. This is a total small business set aside.The associated North American Industry Classification System (NAICS) code is 541611, and the Small Business size standard is $15,000,000.00. A list of contract line item numbers and items, quantities and units of measure, including options, can be found in the attached SOW. Description of requirements can be found in the attached SOW Period of Performance the base period shall be one (1) year from date of award. With options, the total period of performance shall be five (5) year form date of award. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Addendum to 52.212-1 Proposals shall be submitted via a single email with three separate adobe PDF and one MS Excel worksheet formatted attachments as detailed below. All information shall be confined to the appropriate quotation Volume to facilitate independent evaluation. The combined total size for all four files shall not be greater than 5MB. To aid in evaluation, written portions of the quotation shall be clearly and concisely written as well as being neat, indexed (cross-indexed, as appropriate) and logically assembled. Proposals shall be single sided pages (8-1/2 x 11 with 1" margins on all four sides) typed in no less than 12-point type. Failure to fully adhere to the prescribed format may result in your firm's disqualification from the competition. VolumeTitlePage Limitation Cover letterNone I Company, Product and Past Performance5 IITechnical20 IIIPriceNone All quotes must include the following information to be provided in the cover letter: a)Tax Identification Number (TIN) b)Dun & Bradstreet Number (DUNS) c)North American Industrial Classification Systems (NAICS) Code d)Contact name, email address, telephone, and fax number e)Complete business mailing address f)Any unique terms and conditions being quoted   Volume 1 - Company, Product and Past Performance Company and Product Information The vendor must provide the following information: •Offeror company overview and history •Software manufacturer company overview and history (if different from offeror) •Software product overview, history, installed base and growth history (total new, ongoing and retired customers and users by year) •Section 508 Accessibility Self-Assessment Report on product (not subject to page limit) Past Performance The Vendor must provide a description of relevant past performance experience within the last five (5) years in providing products and services similar to those specified in the Statement of Work (SOW); provide a concise summary of your organization's experience; a description of your present and on-going contracts that have a direct relationship to this SOW/requirement. The Government will evaluate relevant past performance of each vendor. Relevant is defined as work similar in complexity and magnitude to the work identified in the SOW. Please include the following information: Specific past performance information must include the following information for not more than three (3) contracts: •Contract/task order, agreement, grant, or subcontract number(s); •Procuring Agency and name of reference point(s) of contact telephone and fax numbers at the Federal, State, Local government or commercial entity for which the contract was performed; •Dollar value of the contract/task order; •Period of performance; •Detailed description of the work performed; •Relevancy of the contract to this proposed contract; •The number, type, frequency, duration and impact of any quality, delivery or cost problems encountered during the performance of the contract and the corrective action taken, if any, and the effectiveness of the corrective action. The Vendor must either provide the above information or affirmatively state that it possesses insufficient relevant past performance that is directly related or similar to the efforts required by the SOW. In cases where a Vendor lacks relevant past performance history, or in which information on past performance is not available, the Vendor will not be evaluated favorably or unfavorably on past performance and a "neutral" rating will be provided to any Vendor lacking relevant past performance history. The government has the right to refer to CPARS. Volume II - Technical This Volume shall address the Vendor's technical understanding of the products and work and illustrate the product's technical capabilities and the Vendor's technical expertise for performing the tasks identified in the SOW. This Volume must demonstrate how the software products offered meet the technical requirements described in the SOW, and how the Vendor can bring unique technical expertise, and best industry practices to bear in performing the tasks requested. This volume must adequately describe the Vendor's technical solution and how it addresses the requirements of the SOW. This volume must describe vendor's plan, milestones, and constraints for delivering the items in the SOW. Page Limitation: The Technical quote submission shall be limited to twenty (20) one-sided pages. Volume III-Price This Volume shall include a Pricing Proposal that includes the base and all option periods. All assumptions or constraints must be identified in the pricing proposal. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a proposal is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). The Government will evaluate FAR Clause 52.217-8 Option to Extend Services. The provision at 52.212-2, Evaluation -- Commercial Items is applicable. Award shall be made to the offeror with the lowest priced technically acceptable offer. Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The following clauses are also incorporated FAR 52.217-8, Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within thirty (30) days. The requirement contains FAR Subpart 52.217-8. The Government will evaluate this option by considering the process proposed for the base period and the option periods with the understanding that should the Government elect to utilize the FAR Subpart 52.217-8 clause, the scope of work and price will be exactly the same, other than period of performance, as the base period or option period being extended. FAR 52.217-9, Option to Extend Term of the Contract (March 2000). A. The Government may extend the term of this contract by written notice to the Contractor at any time within the term of the contract, provided that the Government gives the Contractor preliminary written notice of its intent to extend at least (60) days before the contract expires. The preliminary notice does not commit the Government to an extension. B. If the Government exercises this option, the extended contract shall be considered to include this option clause. C. The total duration of this contract, including the exercise of any options under this clause, shall not exceed 66 months. The following additional clauses are cited: 52.204-7 Central Contractor Registration (APR 2008); FAR 52.209-2 (JULY 2009) Prohibition on contracts with corporate expatriates; 3052.242-72 Contracting Officer's Technical Representative.
