- Usability Study provisions and clauses Statement of Work and eval factors This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is being issued as a Request for Proposal (RFP) under solicitation number 2016-Q-65966 using FAR Part 13.1, Simplified Acquisition Procedures. Any incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89, effective August 15, 2016. Please refer to the attached document for clauses and provisions applicable to this acquisition. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. The full text of all clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. To be eligible to receive an award resulting from this solicitation, the contractor must be registered in the System for Award Management. To register apply via the Internet at http://www.sam.gov. The Centers for Disease Control and Prevention intends to award a firm-fixed-price purchase order for the following service: CLINs 1. Documentation for the Usability Studies of Next-Generation Closed-Circuit Mine Escape Respirators All work shall be performed in accordance with the attached Statement of Work (SOW) Proposals shall include capabilities in accordance with the SOW and Evaluation Factors. Offerors shall include both a technical and cost proposal, and include fully burdened labor rates in the cost proposal. Acceptance and FOB point: Delivery address Period of performance: September 30, 2016 - September 29, 2017 The proposal format is at the discretion of the offeror. Offerors are advised that an award may be made without discussion or any contact concerning the proposals received. Offerors should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial proposals should contain the offeror's best terms from a technical and price standpoint. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: see attached provision FAR 52.212-2. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offers. No telephonic quotes will be processed.
