Description
The Department of Veterans Affairs Medical Center, NCO 9 Network Contracting Office, on behalf of James H. Quillen VA Medical Center, is issuing a sources sought synopsis as a means of conducting market research to identify parties having an interest in and the resources to support this requirement for a firm-fixed priced contract for Installation of Philips Intellivue Monitors. The result of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 238290. Disclaimer and Important Notes: This Sources Sought Notice is for market research purposes only and does not constitute a Request for Proposal/Quotation; and, it is not considered to be a commitment by the Government to award a contract nor will the Government pay for any information provided; no basis for claim against the Government shall arise as a result from a response to this Sources Sought Notice or Government use of any information provided. Failure to submit information in sufficient detail may result in considering a company as not a viable source and may influence competition and set-aside decisions. Regardless of the information obtained from this Sources Sought Notice, the Government reserves the right to consider any arrangement as deemed appropriated for this requirement. BASED ON THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH, THIS REQUIREMENT MAY BE SET-ASIDE FOR SMALL BUSINESSES OR PROCURED THROUGH FULL AND OPEN COMPETITION. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding their submissions. The government will evaluate market information to ascertain potential market capacity to: 1. Provide services consistent in scope and scale with those described in this notice and otherwise anticipated. Secure and apply the full range of corporate financial, human capital, and technical resources required to successfully perform similar requirements. 2. Implement a successful plan that includes: compliance with program schedules; cost containment; meeting and tracking performance. 3. Provide services under a firm-fixed price contract. STATEMENT OF WORK PART A - GENERAL INFORMATION A.1 INTRODUCTION - Mountain Home VAMC is in need of a vendor to add Philips Classic N Intelliview central monitoring for eight beds to the D1 area of building 200 at the James H. Quillen VAMC, TN. A.2 BACKGROUND - the C-Ground ward is being relocated to the D1 area of building 200. This will require additional LAN connections for a central monitor nurse's station along with eight bedside monitors. These must be installed in coordination with the system manufacturer's requirements, NFPA 99, and the National Electric Code. A.3 SCOPE OF WORK - Contractor shall provide all services, labor and associated materials to facilitate as follows: 1. Install Philips Classic N central monitoring with bedside monitor to the D1 area of building 200 as indicated below. a)Install twelve Cat 5/5e/6 cables, wall plates, and terminations between the D-Ground switch and following areas; Nurses station located adjacent room D115; Patient rooms D152, D114, D113, D110, D109, D106, D105, and D101. b)Provide remote solutions hardware for Nurse's station to relocate computer hardware to switch room on D-Ground. c)Install 19" GCX wall channels in eight patient rooms where LAN connections are installed. d)Provide flat panel 23" inch display for central nurse station monitoring. e)Configure Philips switch and Intelliview Classic N software interconnect to existing network. f)Relocate existing Philips equipment for central nurse station monitor and four bedside patient monitors from C-Ground to D1 ward. g)Test and certify network and equipment operation prior to release to facility for patient use. 2. Vendor will perform work IAW all applicable regulations governing medical networks in healthcare facilities (NFPA 99) and National Electric Code. B.1 WORK REQUIREMENTS 1. Vendor will provide all parts, labor, travel, and shipping needed to complete this work C.1 SUPPORTING INFORMATION 1. Place of Performance - James H Quillen VAMC, Buildings 200 2. Period of Performance - Performance needs to begin as soon as possible following the contract award. It is preferred that work be accomplished during the hours of 08:00 AM to 5:00 PM, Monday through Friday. 3. Contractor Furnished Materials - All labor, parts, supplies and tools to complete the work identified. 4. Security Requirements -Per IT Checklist 6500.6 the C&A requirements do not apply and a Security Accreditation Package is not required. Contractor shall identify which employees will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, sub-contractor employees, suppliers and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall be worn above the belt at all times while on Medical Center property. Contractor's employees shall return all I.D. badges to designated personnel at time designated in the task order. 5. The Contracting Officer's Representative (COR) is responsible for the inspection of the work. The Contracting Officer's Representative will be identified by name, telephone number and location. Inspections made by the COR are for the sole benefit of the Government and do not relieve the Contractor of any quality control responsibilities. Submission of Information: Companies having capabilities necessary to meet or exceed the stated requirements are invited to provide information to contribute to this market survey/sources sought notice including commercial market information and company information. Companies may respond to this Sources Sought Notice via e-mail to scheronica.cochran@va.gov. The subject line must specify Installation of Philips Intellivue Monitors. Responses must be submitted no later than August 22, 2016 at 10:00am, Central Standard Time with the following information/documentation: 1. Organization name, address, point of contact, email address, Web site address, and telephone number. 2. Business size: small, small disadvantaged, woman-owned, Hub Zone, serviced-disabled veteran-owned small business, large business, etc. 3. Business type under North American Industry Classification System (NAICS) 238290, which contains a size standard of $15 Million, CAGE Code and DUNS Number. 4. Under which NAICS code does your company usually provide the requirements described in the Statement of Work (SOW)? Please provide rationale for your answer is different from NAICS 238290. 5. Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational and staff capability.
