Description
Solicitation/Statement of Work Health and Human Services intends to procure 7 each, Sensaphone Sentinel Cellular Monitor Temperature Device (or like item) with cellular service. The solicitation will be issued as a commercial Request for Quote (RFQ) in accordance with FAR Parts 12 and 13. This is to be solicited as a 100% Small Business Set-Aside. The applicable NAICS Code is 334512. The business size standard is 500 (Size standards in number of employees). The resultant award will be a firm fixed-price service contract. The award will be for a twelve (12) month base year and four (4) one (1) year option periods. The Statement of Work (SOW) is included as an attachment to this notice. Delivery is required within fifteen (15) days after award. Delivery shall be FOB Destination. All items must be brand name or equal. If the items are not Sensaphone Sentinel Cellular Monitor Temperature Devices, provide the salient characteristics/ specifications of the like item (which must meet the requirements of the SOW). Offerors wishing to submit a quote are responsible for downloading their own copy of the RFQ and for frequently monitoring the site for any amendments. Offerors must be currently registered in the System for Award Management (SAM). To be considered for award, vendors must submit their quotes on the solicitation document posted to FedBizOpps to include pricing. Offerors not submitting a fully completed quote will be considered non-responsive and will not be considered for award. The solicitation number for this requirement is 16-100-SOL-00033. The closing date of the solicitation will be 22 Aug 2016 at 1:00 PM Eastern Standard Time (EST). Any questions must be submitted no later than 17 August, 2016 by 2:00 PM EST. No telephone or fax requests for the RFQ package will be accepted. Failure to respond to the electronically posted RFQ and associated amendments prior to the date and time set for receipt of quotes may render vendor offer non-responsive.
