- Sole source J&A document NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention intends to award a sole source firm fixed price purchase order to George Holdsworth, 871 W. Outer Dr., Oak Ridge, TN 37830, for services to provide consistent criteria and a systematic approach for assessing chemical hazards in the workplace in order to make appropriate risk management decisions to protect workers with potential exposures to these chemicals. This is a 24 month requirement. The capability requirement that makes it a sole source is this data is exclusive and has proprietary data. George Holdsworth is the only source that can provide the data that can meet the needs of the government. For this contract action the government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b) and 10 U.S.C. 2304(c)(1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive quotations; however, all quotations/responses received from the issuance of this notice will be considered by the government. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Any quotation/response should be emailed to Dwight Favors, Contract Specialist at dyf3@cdc.gov by August 11, 2016. If no affirmative responses are received on or before the date stated, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with George Holdsworth, 871 W. Outer Dr., Oak Ridge, TN 37830, as the only source and a purchase order will be issued without any additional notices being posted.
