- Statement of Work Commercial Clauses This is a combined synopsis/solicitation for commercial products prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. It is the quoter's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-88-1 (June 15, 2016) This is a total small business set-aside. The North American Classification System (NAICS) code for this procurement is 541990 –All Other Professional Scientific and Technical Services- with a small business size standard of $15.0 Million. Quotes are due on Wednesday, August 17, 2016, by 3:00 PM Eastern Standard Time and must be sent via email to the attention of Eric Sullivan via email at lrv4@cdc.gov. Hard copies are discouraged but may be delivered via Fed Ex or UPS to Eric Sullivan, Centers for Disease Control and Prevention, Office of Acquisition Services, 626 Cochrans Mill Road, Building 140, Pittsburgh, PA 15236. The Offeror must cite RFQ Number 2016-Q-65818 in the email subject line and include in on the shipping label if sending hard copies. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12 Commercial Items and FAR Part FAR 13 Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM) prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a firm, fixed price purchase order in accordance with FAR Part 13. Proposals must be submitted in two parts: a technical proposal and a pricing proposal. The Centers for Disease Control and Prevention, National Center for Health Statistics (NCHS), National Vital Statistics System (NVSS) has a need to update the HL7 Version 3 Domain Analysis Model: Vital Records (VR DAM) and, in the process, identify any modifications, enhancements, mappings and other documentations that need to be done. The VR DAM presents the public health community with a valid representation of various concepts, relationships, and processes that collectively define the vital records domain, promote semantic interopability across organizations and systems, and lay the foundation for standardized transmission of selected vital events data as work towards electronic health records proceeds. The VR DAM includes activity models of typical birth, death and fetal death processing in the U.S. utilizing flow diagrams to identify the work flow processes, stakeholders and data exchanges that support data collection, record registration, and certified copy issuance. It also includes birth, death and fetal death class models that depict the vital records data captured and the data relationships. The VR DAM is based on the core set of vital records data elements that are on the United States (U.S.) Standard Certificates of Live Birth and Death and the U.S. Standard Report of Fetal Death. Please see the attached Statement of Work (SOW) for a more complete description of the specific tasks to be completed under this contract. After evaluation of the following technical factors, award will be made to the lowest priced technically acceptable offer. Each offer will be evaluated on a pass/fail basis, so offerors are cautioned that they should submit responses in sufficient detail to enable the government to evaluate their capabilities and experience. Evaluation Criteria: 1) Demonstrated expertise in the HL7 standards development process including development; 2) Demonstrated experience with Service Oriented Architecture (SOA) and Representational State Transfer ( REST) interface s; 3) Demonstrated experience with application integration and data modeling as it applies to the exchange of health information using HL7 standards; 4) Demonstrated experience in engaging in health information exchange, and health data integration; 5) The technical proposal must clearly describe and illustrate that the services offered will comply with all specifications listed above and contained within the Statement of Work. Evidence of past performance for the services proposed as well as examples of successful completion of similar work by vendor personnel must be provided.
