- This is a SOURCES SOUGHT NOTICE in support of the PORTLAND VA MEDICAL CENTER PORTLAND, OR AND VANCOUVER CAMPUS OF THE PORTLAND VA MEDICAL CENTER, for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described below. Potential offerors are invited to provide feedback via e-mail to York Barrett at York.Barrett@va.gov. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. All responses are due by August 11, 2016, at 1300 PDT. Potential contractors shall provide, at a minimum, the following information: 1) Company name, address, and point of contact, phone number, e-mail address, and DUNS. 2) Is your firm eligible for participation in one of the following small business programs? The Capabilities Statement for this sources sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding the company's ability to provide the services outlined in the draft SOW below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. DESCRIPTION OF THE REQUIREMENT: Steam Clean for Sterilizer Chambers STATEMENT OF WORK STATEMENT OF WORK (SOW) as of 27 July 2016 Contract Number: Task Order Number: IFCAP Tracking Number:648-16-1-5098-0558 Follow-on to Contract and Task Order Number: 1. Contracting Officer's Representative (COR). Name: Section:FMS Address: Phone Number: Fax Number: E-Mail Address: 2. Contract Title. To clean steam chambers of sterilizers for (3) Getinge 733, and (4) Getinge 433 steam sterilizers. 3. Background. Facility was asked to steam clean the steam chambers of the sterilizers. 4. Scope. Service to clean steam chambers of sterilizers for (3) Getinge 733, and (4) Getinge 433 steam sterilizers. All will be after hours to clean. One at a time. 5. Specific Tasks. 1.Provide chamber cleaning of (3) Getinge 733's in SPS. One at a time. Must coordinate with services to accomplish. These can be completed on dayshift. 2.Provide chamber cleaning of (4) Getinge 433's in the OR's. One at a time. After 6 pm and prior to 5 am hours. Must coordinate with services to accomplish. Note For purposes of this award, Resident Engineer (RE), Project Engineer, and Contracting Officers Representative (COR) are used interchangeably. 6. DEFINITIONS. 4.1"CO" is the Contracting Officer. 4.2 "Contractor" is the contractor performing the contract. 4.3 "POC" is the Point of Contact. 4.4"VA" is the Department of Veterans Affairs. 7. CONTRACTOR RESPONSIBILITIES. 7.1. The contractor shall meet Federal Information Processing Standard (FIPs) 201 for Personal Identity Verification (PIV) badging requirements, IAW (in accordance with) FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel. 7.2The contractor shall furnish all transportation, equipment, tools, and labor to accomplish this requirement. Hand tools, multimeter, laptop, software, or nay item that is needed to troubleshoot and repair the equipment. In some cases, this may be proprietary items. 7.3Contractor employees shall comply with the Standards of Conduct on VA property outlined in title 38 CFR section 1.218, which can be found posted at the entrance to the Medical Center. 7.4The contractor shall be notified by the users of any given machine by phone when a unit is affected. 8.GOVERNMENT RESPONSIBILITIES. 8.2. Restroom facilities. 8.3. VA will provide contractor employees with VA issued badge/identification prior to being authorized to work on the premises. 9.SECURITY REQUIREMENTS. Logical access will be needed for this contract. The contractor must meet FIPs 201 security requirements if any work is to be done on the premises. Work will normally be completed during normal working hours of 8:00AM through 5:00PM, Monday through Friday, excluding federal holidays. Emergency support will be coordinated with the COR. 8.PROJECT LOCATION. This service needed is located at the Portland, OR, VA Medical Center. The address is: Portland VA Medical Center 3710 SW U.S. Veterans Hospital Road Portland, OR 97239 9. HOURS OF OPERATION. Regular hours are 8:00 am - 5:00 pm, local time, Monday through Friday, except for Federal Holidays. Work outside the normal working hours when directed by the VA must be with the written approval of the Contracting Officer unless those hours have been included. Work outside these hours at the request of the contractor shall not result in any increase in price. 10.
