- This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Sources Sought to provide the products identified (Stryker, Model 6254 Evacuation Chair). Please advise if you carry this product or a suitable "Or Equal" and if so indicate the Size and Social Economic status of your company. EMAIL INFORMATION TO : Steven.Turner3@VA.GOV STATEMENT OF WORK Stryker, Model 6254 Evacuation Chair or Equal Function/performance requirements: The evacuation chair shall be able to be strap patients into the chair and support a single patient weighing up to 500lbs. The chair shall be able to move down the hallway and stairwells with minimum exertion by the medical staff and allow them to have control over the chair while going down stairwells (e.g. large wheels and extendable upper control handle). The evacuation chair shall meet emergency response guidelines established to support the Americans with Disabilities Act. The evacuation chair shall be able to be folded and hung on the wall with a mount and cover provided by the manufacturer. The evacuation chair shall include an oxygen cylinder holder that will hold a 'D'/ 'Jumbo-D' size medical oxygen cylinder and will attach to the chair and will not interfere with the transporting a patient. Pop-out handle bars to be used by an additional staff member to help guide the chair down the stairwell while facing the patient. A minimum of 15-year warranty on defects of materials and workmanship shall be provided. The chair should be lightweight, preferably Aluminum construction, and made with materials that can be cleaned and disinfected to be able to return it back to service after use. The manufacturer/contractor providing the evacuation chair shall send a representative to provide in-house training on how to use the evacuation chair and provide demonstrations of how to use it. The evacuation chair shall provide foot rests so that the leg can be extended out and not only at a 90 degree angle. The evacuation chair shall require minimal maintenance to maintain the chair in operation condition (i.e. grease-free components). The track that allows the chair to slide down the stairs shall have a tongue and grove friction based tread to prevent slippage or speeds that will make it difficult to control. Delivery: Evacuation Chairs and accessories shall be shipped FOB to destination and be received within 30 days of receipt of award: Northern Arizona VA Health Care System Warehouse Building 20 500 Hwy 89N Prescott, AZ 86301 Packaging and marking, items must be adequately packaged to prevent damage during shipping handling, and storage. Any damaged during shipment shall be replaced at the Vendor's expense. Outer packages shall contain the complete contract number, obligation number, and shipping address. A packing list/slip shall be enclosed with shipment and shall include delivery order number, items and quantity ordered, and items and quantity shipped.
