General Conditions Drawings and Specs Solicitation Tennessee Air National Guard 240 Knapp Blvd. Nashville, TN 37204-1502 615.660-8889 DATE: Aug 1, 2016 TO: Contractors FOR: Remoldel Bldg 720 PROJECT NUMBER: BKTZ132021 Subject to Funding. No contract award will be made until appropriated funds are made available. AFARS 5101.602-2(a) (ii) (A) and National Guard Acquisition Manual (NGAM) 1.602 ACKNOWLEDGEMENT OF AMENDMENTS: I/we have received and considered the following Amendment(s) in the submitted proposal for this project. Watch your email in the event an amendment is sent out. None () #1(X) #2() #3() #4() #5() ALL WORK TO BE PERFORMED IN ACCORDANCE WITH THE ATTACHED Plans and Specifications. Contractors are required to track project progress on AF 1477 or approved equivalent. Progress Schedule FAR 52.236-15: The contractor is responsible for submission of AF Form 3064 for the Contract Progress Schedule within 7 calendar days from issuance of Notice to Proceed. Thereafter, the contractor and contracting officer's representative (COR), are responsible for submission of AF Form 3065, Contract Progress Report. Both of these documents will be submitted to the contracting office. North American Industry Classification (NAICS) is 236220 with a small business standard of $36,500,000.00 DEPARTMENT OF LABOR: Davis Bacon General Decision Number: TN160084 05/13/2016 TN84 Superseded General Decision Number: TN20150084 Magnitude of Construction: Between $100,000 and $250,000. There are drawings for this project. All work is to be performed in accordance with the Plans and Specifications. PERFORMANCE PERIOD: 120 with receipt of signed Notice to Proceed by the contracting officer. PROPOSAL ACCEPTANCE PERIOD: The proposal acceptance period is 7 days from date of receipt of proposals. PERFORMANCE & PAYMENT BONDS: NOT REQUIRED. In accordance with FAR 52.228-15, Contractors are reminded that any contract awarded over $30,000 requires submission of a Payment Bond and a Performance Bond is required when contract awards in excess of $150,000. Site Visit FAR 52.236-3, Site Investigation: Aug 9, 2016. 9 A.M Central Time. Contractors in attendance of meet at Tennessee Air National Guard 118th AW Nashville, TN. SITE VISIT IS NOT REQUIRED TO SUBMIT PROPOSAL FOR THIS REQUIREMENT. QUESTIONS: Submit question by email to bryan.k.white10.mil@mail.mil before 22 Aug, 2016 not later than 1:00PM Central Time. Questions submitted after this timeframe will be addressed with the awardees only. AWARD OF CONTRACT FAR 15: Award of this contract will be based upon lowest price technically acceptable to the government; see evaluation factors. The evaluation criteria for this project will be: LOWEST PRICE TECHNICALLY ACCEPTABLE in Accordance with FAR 15.101-2. Evaluation factors will be: technical, past performance and price. Technical Factors: Meets or exceeds requirements set forth in the Plans and Specifications. Please see clause 52.211-6 Brand name or equal when submitting proposal when it comes to the materials used in proposal. Materials must meet or exceed standards of brand name mentioned in PWS. Past Performance: NOT REQUIRED Current- Any recent contracts similar in nature Relevant- Any contracts relevant in nature of this requirement. No past performance submitted will not be evaluated favorably or unfavorably. Price: Lowest price that meets the evaluation factors above. Clauses: FAR 52.204-10 REPORTING: Executive Compensation and First Tier Subcontract Awards (Jul 2010). Is applicable to this order. The following are the codes needed to accomplish the reporting TAS 57, Agency 3840 1) FAR 52.223-6 Drug-Free workplace http://www.dol.gov/elaws/asp/drugfree/screenfq.htm Drug-Free Workplace is applicable for awards over the $150,000 (SAT) and is applicable to this task order, and is required under $150,000 by this solicitation. 2) FAR 52.222 -6 Davis-Bacon Act: The WH-1321 poster can be obtained at no charge from offices of the Wage and Hour Division. The poster can also be downloaded from http:// www.dol.gov/whd/regs/compliance/posters/fedprojc.pdf. The poster shall identify the name, address, and telephone number of the Government officer responsible for administering the contract in order to inform workers to whom they may submit complaints or raise questions concerning labor standards. In the absence of such posted information, any person who wants to determine if the project is covered should contact the federal agency funding or assisting the project or the Wage and Hour Division. 3) FAR 52.236-5, Material and Workmanship is applicable to this task order. 4) FAR 52.236-9, Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements is applicable to this task order. 5) FAR 52.236-13 Accident Prevention: In accordance with PROJECT SAFETY construction contractors are required to use the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1. INFORMATION REGARDING AVAILABILITY OF UTILITY SERVICES: It has been determined the government will provide the contractor all utilities available currently on site at no cost to the contractor. Offerors will take this into consideration when computing an offer. 11) FAR 52.246-21 : Warranty of Construction is applicable to this request for proposal. Written warranties are required for this award. 12) DFARS 252.232-7003: Wide Area Work Flow (see website - https://wawf.eb.mil/) is applicable to this order with supporting documentation uploaded with invoiced pay request. Construction Payment terms are Net 14 Days with final payment at Net 30 Days. DFARS 252.232-7006 will have codes for WAWF submissions to be completed. 13) PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY. (September 2002) This special contract clause is Not Applicable To this Order. 14) FAR 52.211-6, Brand Name or Equal: If an item in this solicitation is identified as brand name the contractor may assume this is brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy or exceed the Government's needs. (a) The salient physical, functional, or performance characteristics that this product has must meet or exceed specifications. The contractor or vendor is responsible to supply information to the contracting officer with Proposal. (b) To be considered for an acceptable product for equal products, including equal products of the brand name by the manufacturer, the product must meet or exceed the salient physical, functional, or performance characteristics and must be approved by the Contracting Officer. Each equal item must be clearly identified by: (i) Brand name, if any; (ii) Make and model number, (iii) Color, (iv) Descriptions (v) warranty and servicing information (vi) specifications, (vii) Other features... (c.) To properly support the idea of competition and rejecting a request for a brand name item the contractor is encouraged to supply more than one equal item that accomplishes the needs. A general rule is three (3) like items unless the contractor would like to support more choices to the government. (d) Include descriptive literature such as illustrations, drawings, or a clear reference to furnished descriptive data, pictures, and other pertinent information to the Contracting Officer; and CLEARLY DESCRIBE any and all modifications to the alternative product. Also CLEARLY DESCRIBE any changes the proposed product makes to plans to make in installation / shipping. Mark up any and all descriptive material to clearly show any and all modifications.
