PDF Copy of the Combined Synopsis/Solicitation (i) This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. (ii) The Requisition number is: OS176529 and is issued as a Request for Quote (RFQ). (iii) This solicitation document incorporates all mandatory commercial item provisions and clauses that are in effect through Federal Acquisition Circular (FAC) 2005-80. (iv) The North American Industrial Classification System (NAICS) code is 812930, and the small business size standard is $35.5M. Please identify your business size in your response based upon this standard. (v) The PROGRAM SUPPORT CENTER (PSC) is soliciting Seven (7) unreserved parking spaces at the Parking garage that near 700 Stewart Street, Suites 06103 & 11101, Seattle, WA 98101. (vi) Description of Requirement: The U.S. Department of Health and Human Services (HHS), Program Support Center (PSC), on behalf of the Office of Medicare Hearings and Appeals (OMHA) has a requirement to Lease Seven (7) Unreserved Parking Spaces for Senior Leadership in its Seattle Field Office at 700 Stewart Street, Suite 06103 & 11101, Seattle, WA. Item # Description Qty. Monthly Price 12 Month Price 1 Monthly Parking passes for Senior Management Employees 7 $ $ TOTAL AMOUNT: $ (vii) Period of Performance: The period of performance shall be from August 22, 2016 through August 21, 2017 plus one (1) 6 Months Option Period (08/22/2017 - 02/21/2018). (viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items (APR 2014) applies to this acquisition. (ix) The provision at FAR 52.212-2, Evaluation-Commercial Items (OCT 2014) applies to this acquisition. The Government will award a contract resulting from this synopsis/solicitation to the responsible offeror who provides the Lowest Price Technically Acceptable. (LPTA). Offerors must meet all evaluation criteria to be considered technically acceptable. The evaluation criteria are the minimum requirements described in (xii) as follows: location is 700 Stewart Street, Suites 06103 & 11101, Seattle, WA 98101. (x) Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (MAR 2015), with its offer or acknowledge its listing in the System for Award Management (SAM) website. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (MAY 2015), applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (JUN 2016), applies to this acquisition. This Statement is for Information Only: It is not a Wage Determination Employee Class Hourly Wage Fringe Benefits 31260 - Parking and Lot Attendant $10.07 $3.31 (xii) Statement of Need: STATEMENT OF NEED - MONTHLY PARKING PURCHASE I. Background The Office of Medicare Hearings and Appeals (OMHA), under direct delegation from the Secretary of the Department of Health and Human Services, administer hearings for the Medicare program. OMHA Seattle Field Office Senior management needs monthly parking in order to efficiently administer this Medicare program. II. Scope OMHA requires seven (7) monthly parking passes for senior management employees. The parking garage must have 24 hours per day, 7 days per week availability. The parking garage must have a secure and monitored entry and exit point by key or access card. The parking garage must be within 0.20 miles of 700 Stewart St., Suites 06103 & 11101, Seattle, WA 98101. The parking spaces can be indoor or outdoor non-reserved spaces, and can be covered or non-covered spaces. III. Evaluation Criteria Due to security concerns for the judges, distance is the most important criteria, followed by cost. IV. Period of Performance Base Year (08/22/16 - 08/21/17) + 6 Months Option (08/22/17 - 02/21/18) V.
