We are seeking information for market research purposes only. The Government will not pay for any information solicited. The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following item(s) for the Kernersville Health Care Clinic, 1695 Kernersville Medical Parkway, Kernersville, NC 27284: HM39SPU-TVAHoverTech 39' Single Patient Use HoverMatt, 10 ea. per box12 BX Only Exact Brand Name items will be considered for this request. Order against: 1 FSS Contract Number: V797D-40330 Name of Proposed Contractor:Hovertech International Street Address:513 S Clewell Street City, State, Zip:Bethlehem, PA 18104 Phone:800-471-2776 3.Description of Supplies or Services: The estimated value of the proposed action is $ 10497.60 Hovermatts, with Inflation Regulator and Cart, for the Kernersville Health Care Clinic. Brand Name Only requested to maintain standardization throughout the facility. 4.Authority: ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. (FAR 8.405-6(a)(1)(i)(C) 5.Rational/explanation to support cited authority: The staff at the hospital has been trained and has extensive experience with this specific brand of equipment, and the physicians have requested this specific brand for use on all of their patient procedures in the hospital. In the interests of patient safety, it is of the utmost importance to maintain standardization of this equipment throughout the facility. Brand Name Only requested to maintain standardization. Market Research revealed 1 Small Business vendor on FSS Schedule 65 II A. Search of FPDS history shows a possibility of 2 Small Business vendors. A Sources Sought will be posted to FBO to determine if there are any SDVOSB/VOSB vendors that carry the items. 6.BRAND NAME ONLY: This buy is not estimated to exceed $25,000 and this documentation will not be posted along with the RFQ on e-Buy, as required by FAR 8.405-6(b)(3). 7.Note that this determination satisfies the requirements of FAR 8.405-6.
