- STATEMENT OF WORK (SOW) ThinPrep 2000 May 10, 2016 Phoenix Veterans Administration Health Care System 650 E. Indian School Road Phoenix, AZ 85012 Transaction Number: 644-17-1-034-0016 POC: Catherine Maniaci Histopathology Supervisor Pathology & Laboratory Medicine Service (P&LMS) This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Please advise if you carry this product are a suitable "or equal". Also include company socio/Economic status and size. If you have any questions or concerns in reference to the Parts listing please provide as well. EMAIL INFORMATION TO : james.vercoe@va.gov Part Numbers offered are for example only. 1.0 Scope of Work: The Phoenix VA Health Care System, Pathology and Laboratory Medicine Service (P&LMS), Histology section uses the Hologic ThinPrep Pap Test to process pap smear slides that we get from the Well Women Clinic. The ThinPrep pap test is a liquid-based preparation, improving specimen adequacy and significantly increasing test sensitivity. The test is the first of its kind and since its introduction; it has contributed to a further 28% reduction in invasive cervical cancers in the US. The ThinPrep Pap Test is FDA approved 2.0 Tasks: The Contractor shall provide all products, labor, and materials according to the SOW Specifications. The items required must meet or exceed the specifications listed below. General Requirements; the contractor shall provide the following: 1.Specifications set forth in this proposal and attached quote are specific to the ThinPrep 2000 instrument. No substitutions are available as Hologic is the only manufacturer of the ThinPrep 2000 and its consumable products. 2.The offerer shall meet or exceed the minimum requirements and shall be held responsible for the performance and overall quality of the consumables purchased by the end-user. 3.The consumables purchased will be new and compatible with the ThinPrep 2000 model. The manufacturer of record is defined as a company whose main occupation is the manufacture for sale items of equipment and which: a.Maintains a factory production line. b.Maintains a stock of replacement consumables. c.Maintains consumable literature (package inserts). d.Maintains and supplies published and distributed descriptive literature that directly relates to the consumables purchased. 4.The vendor should have the ability to fill emergency supply requests when requested. 5.The vendor will pay shipping fees for all reagents and consumable items ordered. 6.Any additional cost such as industrial funding fees, special shipping and handling fees or other usage fees must be clearly indicated in the purchase price quote. 7.All items quoted must be F.O.B. destination. All items shall be shipped to the following location: Phoenix VA Health Care System, Histology Dept (113), 650 E. Indian School Road, Phoenix, AZ 85012 M-F, 8am-4pm, excluding holidays. 2.1 System Requirements; the contractor shall provide the following: N/A for this request. 2.2 Instrument Requirements; the contractor shall provide the following: N/A for this request. The PVAHCS has purchased and is in possession of the ThinPrep 2000 instrument. 2.3 Supply Requirements; the contractor shall provide the following: 1.All necessary accessories required for operating the ThinPrep 2000 should be listed as an addendum in the pricing schedule. 2.All items purchased must be compatible with the ThinPrep 2000 instrument. 3.Contract will allow flexibility for future expansion of testing platforms based on the end-users discretion. 4.Contract will include base year (12 months) and four (4) one-year options. 2.4 Repair & Maintenance Services; the contractor shall provide the following: N/A for this request. 2.5 Pre-Installation Assessment; the contractor shall perform the following: N/A for this request. 2.6 Support Services: The contractor shall perform the following: 1. The manufacturer/vendor should have the ability to provide service sufficient to meet the needs of the user facility Monday-Friday 8:00am to 4:30pm Arizona time. 2. In the performance of this contract, the vendor shall take such safety precautions as the Contracting Officer may determine to be reasonable necessary to protect the lives and health of occupants in the building. The contracting Officer shall notify the vendor of any safety issues and the action necessary to correct these issues. Such notice, when served on the vendor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the vendor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work and hold the vendor in default. 3.0 Mandatory Value-Added Characteristics: These characteristics are mandatory, and will be considered in the best-value determination. Value added characteristics include, but are not limited to: 1.Provide utility requirements (electrical, media disposal, etc.) which should include all necessary SDS, OSHA and safety documents. 2.Provide environmental requirements (heat/cool, humidity, light, etc.) and recommendations for ThinPrep 2000 storage. 3.The manufacturer should define maintenance requirements and indicate what maintenance is performed by the user and what is not.
