Description
This acquisition is a Prospective (Intended) 100% set aside for Small Business. If Two or more interested Service Disabled Veteran Owned Small Business (SDVOSB) respond, then the solicitation to acquire services described below, shall be 100% Set-aside to SDVOSB. If there are no SDVOSB or VOSB responses, then this requirement will be available to all interested Small Businesses. The NAICS Code is 237310 (Highway, Street, and Bridge Construction) $33.5 size standard. Please note: That this is not a request for quotes or proposals; this is a pre-solicitation notice to inform all interested Small Businesses that the VA National Cemetery Administration, Pacific District as directed by the VA San Joaquin National Cemetery, will solicit a Set-Aside proposal. This Request for Proposal (RFP) shall be under No: VA786-16-R-0316. A RFP solicitation is currently not available. The intent of an awarded contract shall be to extend the Cortege Lane. Project: 913-MM-FY16-008 The evaluation factors are as follows technical; Past Performance when combined shall be significantly greater than Price. BACKGROUND: The VA National Cemeteries are a resting place for our nation's heroes and as such, the standards for management, maintenance and appearance of these cemeteries have been established by the VA National Cemetery Administration (NCA) to reflect this nation's concern and respect for those interred there. NCA mission is to honor veterans with a final resting place and lasting memorials that commemorate their service to our nation. National Cemeteries are National Shrines. The standards of maintenance and appearance at these cemeteries shall reflect this nation's concern for those interred. PROGRAM OBJECTIVES: The purpose of this Pre-solicitation is to obtain a contractor to support the National Cemetery administration. Project Title: Extend Cortege Lane All work shall be performed in accordance the Description/Specifications/Statement of Work and drawings contained in this contract. SCHEDULE OF SERVICES: Contract Line Item Description Quantity Unit 0001Extend Cortege Lane1Job SCOPE OF WORK GENERAL SCOPE OF WORK: The Contractor shall furnish all labor, materials, equipment and supplies necessary to perform the work to extend the Cortege Lane at the San Joaquin Valley National Cemetery, 32053 W. McCabe Road, Santa Nella, CA. 95322 1.Contractor to very all conditons, grades and height of walls before submitting proposal. 2.Demolish angled portion of sidewalk, demountable curb and retaining wall. 3.Connect new retaining wall to existing. Match stone veneer. 4.Install new sidewalk(match existing) with stamped concrete apron with demountable curbs. GENERAL REQUIREMENTS All work will be coordinated with the Contracting Officer's Representative (COR) and/or the Cemetery Director. Work will be conducted in such a manner to minimize the impact on the Cemetery operations. All work will be accomplished during normal business hours (8:00 am to 4:30 pm) Monday through Friday, except Federal Holidays, at a time determined by the COR. Only the Contracting Officer (CO) has the authority to make changes to the contract that affects price and/or time. Cemetery Director: Margaret Ayres TEL: (209) 854-1040 COR: Steve Brand Foreman TEL: (209) 854-1040 General Notes: 1.Prior to submission of any bid, the Contractor shall perform a thorough field survey of the existing conditions and features. Any conditions which may cause significant deviation shall be brought to the attention of the Contracting Officer or his/her Representative (COR) for clarification prior to submission of the bid. 2.Submittals: Prior to installation, the contractor shall submit for approval the manufacturer's literature and specifications for ALL materials being installed. Shop drawings will be required for the viga and caps indicating sizes and connections. 3.Construction Coordination: a.Coordination of Construction with Cemetery Director/COR: The burial activities at a National Cemetery shall take precedence over surveying activities. The Contractor must cooperate and coordinate with the Cemetery Director, through the COR, in arranging the schedule to cause the least possible interference with Cemetery activities in actual burial areas. b.The Contractor is required to discontinue his work sufficiently in advance of Easter Sunday, Mother's Day, Father's Day, Memorial Day, Veteran's Day and/or Federal holidays, to permit him to clean up all areas of operation adjacent to existing burial plots before these dates. c.Cleaning up shall include the removal of all equipment, tools, materials and debris and leaving the areas in a clean, neat condition on a daily basis. Contractor is responsible for disposing of debris off site. 4. The Contractor shall assume sole responsibility for safety of all persons on or about the construction site, in accordance with applicable laws and codes. Guard all hazards in accordance with the safety provisions of the latest manual of accident prevention published by the Associated General Contractors of America and OSHA. 5. The Contractor shall guarantee that all work done under this contract will be free from faulty materials or workmanship and hereby agrees to repair or replace without additional cost to the VA and to his/her satisfaction all defects or imperfection appearing in said work within a period of one year after the date of final acceptance by the VA of all work done under this contract. 6. Matters related to safety, and any actions of the Contractor, shall meet all safety requirements of San Joaquin Valley National Cemetery Safety Officer, Department of Veterans Affairs, OSHA, and the State. It is incumbent upon the Contractor to be familiar with these requirements. "Safety" shall also include the Contractor having a safety representative who maintains regular and routine contact with the Safety Officer at RNC. Contractor shall provide training of the contractor's and subcontractor's workers as necessary to meet these requirements. 7.Repair and replace any existing structure to existing condition when altered or damaged by the construction. Completion Time: Ninety (180) calendar days from date of issuance of Notice to Proceed (NTP). (End of Work Statement)
