Description
Representations and Certifications should SAM.gov not contain updated information prior to proposal submission. Fillable suggested Pricing Table for Task Order One (1) Pricing Table in Excel Solicitation document with links, and without attachments. Questions and Answers from prior to July 21, 2016. The original RFP closing date has been extended to August 15, 2016 and the question submission period has been extended to July 25, 2016. Please see the amended Description below. Also see the attached amendment to the RFP with details of the questions received thus far, and answers to those questions. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. The solicitation is being issued under FAR Part 15, "Contracting by Negotiation," and is full and open to business of all sizes. Evaluations will be made using Best Value source selection criteria. Accordingly, technical capability is weighted most important in the evaluation of offers. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-88 effective 15 June, 2016. The Government intends to award an IDIQ, Fixed Price Type Contract with four (4) twelve month Options to Extend the Contract. The National Institutes of Health, Office of Purchasing and Contracts, is seeking a contractor to provide further development of the disability assessment instrument prototype, premised upon Item Response Theory (IRT) and Computer Adaptive Testing (CAT) technology, called the Work Disability Functional Assessment Battery (WD-FAB). See the Statement of Work for specific requirements, attached herein. Questions regarding this solicitation may be submitted to the Contract Specialist by email to Lisa.Schaupp@nih.gov prior to 11:00 a.m. ET, July 25, 2016. Questions will be answered as a modification to this solicitation and posted prior to 5:00 p.m. ET July 26, 2016. Complete proposals must be submitted by 10:00 a.m. ET August 15, 2016 to the Contract Specialist by email at Lisa.Schaupp@nih.gov.
