Description
Synopsis: Wilford Hall Ambulatory Surgical Center (WHASC), JBSA-Lackland, TX is announcing its intent to issue Request for Quotes (RFQ) to up to three (3) firms for the establishment of Blanket Purchase Agreements (BPAs), for Human Cornea Tissue for the procedures known as Penetrating Keratoplasty (PK), Descemet's Stripping Endothelial Keratoplasty (DSEK), or Descemet's Membrane Endothelial Keratoplasty (DMEK) in support of the 59 MDW Ophthalmology Clinic. This requirement is being procured under Simplified Acquisition Procedures (SAP) of the Federal Acquisition Regulations (FAR) Part 13. The NAICS Code for this procurement is 621991Blood and Organ Banks. Under the proposed agreement(s), the BPA contractor(s) will provide the requested Human Cornea Tissue identified by the requesting activity. As requested by the Authorized BPA Caller, the Contractor will be required to deliver the requested cornea tissue at the time identified/specified at the time of the call - normally 48 - 72 hours prior to procedure, but in some instances a more expedient delivery might be required. The Authorized BPA Caller has full authority to determine the expediency of the request and the type of cornea tissue required. Each BPA Call shall not exceed $25,000.00. The period of performance for the proposed BPA is five years or the master call limit of $900,000.00. The interested contractors must be registered with the System for Award Management (SAM) prior to ward, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Failure to register in SAM may preclude an agreement being made with your company.
