- Sierra Army Depot located in Herlong, California has a requirement for Optometry Services to include 4 option years. Performance to begin approximately 30 days after award, on or around 1 September 2016. This procurement is set-a-side for small business concern. The North American Industrial Classification Code (NAICS) is 621320. The related small business dollar amount is $7,500,000.00. Solicitation is being conducted under FAR Parts 12 & 13.5, Full & Open Competition. All responsible sources may submit a quote, which shall be considered. In accordance with FAR 5.102(d), availability of this solicitation will be limited to the electronic medium. The anticipated posting of this solicitation is Wednesday June 29,2016. All contractual and technical questions must be submitted in writing and may be emailed or faxed to 530-827-4722, Attn: Lissa Kaarbo where the answer will be posted to FBO.gov 7 days prior to the closing of this solicitation. No telephone questions will be accepted. Notification of any postponements and any amendments to this solicitation will only be posted on this website. It is the sole responsibility of the offeror to continually view this website for any amendments made to this solicitation. Service requested: Eye Exams Prescription eye wear fitting Single Vision Clear Lens Bi-Focal Clear Lens Tri-Focal Clear Lens Prescription Safety Frames Lens Tinting Period of Performance: 12 month base year with a potential of 4 option years. ** This requirement does require past performance questionnaires to be filled out PRIOR to submission by references only and need to reflect similar work experience.
