- This amendment is a response to questions received as part of this RFQ: Q1)what all do you need returned besides the vendor questionnaires to have a complete bid. I see a blank for the price per item in section 1.3.8 but I do not see a signature or information page for our company. A1) This is where we are asking what dollar value per item that PRDS/PMB/DLS will receive for each piece of property to be received by the vendor for recycle. As to the signature and/or company information, this should be in the overall solicitation return back to Mr. Miller from the company I would think, if not, then that information can be entered into a cover page I would think. Q2) Under section 1.3.11 concerning hard drives. Will we be required to shred/destroy all hard drives, or, is wiping to DOD or NIST standards acceptable? I am fine either way but would prefer to wipe if value justifies it. A2) Destruction is required. The government does not want any PII information out there in the hands of the General Public. Even though we "wipe" before it leave the Institute or Center and then check again at the GDC, we still find hard drives that say they are wiped but are not, or not wiped properly. If a particular hard drive due to value is requested to be wiped only, we would need to know the QA process of the company to ensure the hard drives are tripled checked so that no PII information is released before we would even consider allowing it to be wiped only. Q3)Under section 3.2 Liability, will we be responsible for items once they have left NIH custody as far as repair or replacement. we do not use standard care when dismantling an item for recycling. i would assume this just means property at your locations used when we are picking up items. A3) That is correct, this applies mainly during the time the vendor truck is located at the GDC Warehouse if any damage were to result due to the error of the vendor driver or if the truck floor gave way, etc. Q4) Under terms and condition section 52.212-4 Invoicing Guidance. it asks us to provide a sample invoice. Is this correct? i do not think the contractor will be invoicing NIH on this contract. A4) Under this contract there should be no reason for GDC or NIH to ever get an Invoice from the vendor. This was included as a pre-approval for invoices. Q5) Could we get a list of of items that are recycled monthly? A5) Shakers Water Baths Microscopes Flat Screen Monitors (21" to 23") PCR machines Printers Analytical Balances Centrifuges (mini, table top, large) Spectrophotometers DNA Synthesizers Computers (desktops) Laptops Incubators Geiger Counters Thermal Cyclers Hot plates
