Response Date: 27 June 2016 This is a SOURCES SOUGHT NOTICE for informational/market research purposes only. THIS IS NOT A REQUEST FOR PROPOSALS, QUOTATIONS OR BIDS, NOR A SYNOPSIS OF A PRROPOSED CONTRACT ACTION UNDER FAR SUBPART 5.2. Seattle District USACE requires medical testing services for their employees and for safety/readiness purposes. Medical testing and examinations include pre-employment and clearance examinations of current employees to meet regulatory requirements based on employee job descriptions, exposures, and job tasks. USACE uses the guidelines of CFR 1910 and EM-385-1-1. All medical reports/results require a review by a doctor that is certified or board eligible in Occupational Health. Services are required for approximately 100 employees at the following Seattle District locations: Albeini Falls Dam, 2376 East Highway 2, Oldtown, ID 83822-9243; Chief Joseph Dam, Highway 17 and Pearl Hill Rd, Bridgeport, WA 98813-1120; Libby Dam, 17877 Mt. Hwy 37, Libby MT 59923-7828; Howard Hanson Dam/Mud Mountain Dam, 30525 SE Mud Mountain Rd, Enumclaw, WA 98022-8010; Lake Washington ship Canal, 3015 NW 54th St., Seattle, WA 98107-4299; Federal Center South, 4735 East Marginal Way South, Seattle, WA 98134-2388 Interested parties must submit a capabilities statement containing the following information: Vendor name and address and CAGE code; a list of locations at which they would be available to provide services stating the distance in miles from their office to each location; a statement of availability to provide on-location services; to all/any locations, stating serviceable locations. Vendors will submit professional credentials such as their Certificate in Occupational Medicine from the American Board of Preventative Medicine (ABPM) or the documentation proving they are Board eligible per https://www.theabpm.org/certification.cfm. Submit responses to this announcement as soon as possible, but no later than 2:00 pm (Pacific Time) on 27 June 2016 to Sara Edwards, Contract Specialist, by email: sara.edwards@usace.army.mil. All interested firms must be registered in SAM (https://www.sam.gov) and remain current for the duration of the contract to be eligible for award of Government contracts. NO SOLICITATION IS CURRENTLY AVAILABLE. This is not a request to be placed on a solicitation mailing list and is not to be construed as a commitment by the Corps. The purpose of this notice is to gain knowledge of potentially qualified Small Businesses (SB),8(a), Small Disadvantage Business (SDB), Woman Owned Business, Service-Disabled Veteran Owned Small Business (SDVOSB), or HUBZone sources and their size classifications relative to the North American Industry Classification System (NAICS) code 621111 - Offices of Physicians (except Mental Health Specialists). The small business size standard is $11 million. The Federal Service Code (FSC) Q403. Responses to this sources sought announcement are not adequate response to any future solicitation announcement. The results of this sources sought are in effect for a period of one year from the date of this notice. No reimbursement will be made for any costs associated with providing information in response to this or any future announcement.
