Description: SERVICE AGREEMENT FOR AN ARGON PLASMA COAGULATOR SYSTEM (I)This is a combined synopsis/solicitation for commercial commodities prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA247-16-Q-0635 (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85 effective 01/04/16. (IV)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (V)The Network Contracting Office 7-SAO EAST intends to award a firm-fixed price service contract for Argon Plasma Coagulator System. (VI)Located at the Atlanta, VAMC, 1670 Clairmont Rd., Decatur, GA 30033. (VII)The North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 500 employees. This procurement is unrestricted to and for all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. All questions or inquires must be submitted no later than 7:00 AM EST on July 05, 2016. No phone calls will be accepted. The Gastroenterology Service is required to replace its current argon plasma coagulator system has aged beyond its current life expectancy of seven years. 2.0SCOPE The argon plasma coagulator system and electrosurgical unit shall be as specified and meet the technical specifications listed below. All equipment shall be new. The company shall deliver the equipment to the Atlanta VA. The company shall provide all materials, necessary for delivery. Installation shall be completed by Atlanta VA's personnel. All parts shall be manufactured to meet state and local regulations. 3.0VENDOR/CONTRACTOR REQUIREMENTS\ The vendor/contractor shall ensure Argon Plasma Coagulator System meets specifications. 4.0TASKS The contractor shall complete the following tasks in support of this Delivery: Delivery is by a third party entity i.e. USPS, UPS, FedEx, etc. 5.0DELIVERABLES The vendor/contractor agrees to the following deliverable dates: a.No installation provided by vendor. b.Vendor shall deliver the equipment within (30) days after the award to the Atlanta VA Medical Center Warehouse (no installation). 6.0PERIOD OF PERFORMANCE $ COSTING See Table Below 9.0. 7.0PLACE OF PERFORMANCE The contractor shall provide all parts, user guides and service manuals to VA Atlanta, 1670 Clairmont Road Decatur, GA 30033. 8.0CLASSIFICATION/SECURITY Security clearance is not required. Vendors will not install equipment/software or need system access. 9.0 SALIENT CHARACTERISTICS: "Must have Argon Plasma Coagulation "Unit shall be compatible with Olympus endoscopes. "Plasma unit must be equipped with an automatic adjustment control which adjusts the argon plasma regardless of the impedance of the overall system (plasma regulation). The Precise APC mode creates a superficial coagulation effect using a low-energy output per unit of time. *The vendor must provide (exact match only and equipment will be new). The Argon Plasma Coagulator Unit shall meet the above specifications or equivalent.
