Description
Digital Video Capture Module for Ear, Nose, Throat (Otolaryngology) This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is issued as a Request For Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). Please see the following Description below: Description The Contractor shall provide material for delivering; and training and support for using, configuring, integrating, and programming--the following equipment and supplies and services: PENTAX: Digital Video Capture Module: VA MDNS: Computer - Medical Equipment - Monitor CSN: 6530-440019; LOCAL STOCK NUMBER: 9200D TOTAL: EQUIPMENT AND SUPPLIES: Configuration, Training, and Support for the ENDO-Portal Stations: TOTAL: TRAINING/SUPPORT FOR ENDO-PORTAL STATIONS: Training and Support for DICOM: TOTAL: TRAINING/SUPPORT FOR DICOM: Plus Trade-In Value for Six Computers: GRAND TOTAL: Quantity Six (6) Six (6) One (1)Unit (Each)Price NOTE: Offerors submitting quotations on Equal items shall provide the Manufacturer's Name, Brand and Part Number for each item providing a quotation on. (vi) To be technically acceptable, the products quoted shall be "Brand Name or Equal to" PENTAX. (vii) Delivery FOB Destination to: Richard L. Roudebush VA Medical Center, 1481 West 10th Street, Indianapolis, Indiana 46202-2803. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, FAR 52.209-5DEV; 52.225-25; 52.216-1 (fill in: Firm Fix Price contract will be awarded), 52.232-2, VAAR 852-215-70, 852.233-70; 852.233-71 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (o) of this provision. SUMMARY STATEMENT OF WORK STATEMENT I.SCOPE OF WORK: The Contractor will provide equipment and supplies (material), and training and support for six (6) Digital Video Capture Modules and Computer Stations for Ear, Nose, Throat (ENT) video scopes and Ontolaryngology services at the Richard L. Roudebush VA Medical Center, 1481 West 10th Street, Indianapolis, Indiana 46202-2803. II.CONFORMANCE STANDARDS: All configuration, integration, and training support services provided under this contract must be performed in conformance with the Federal regulations and AAVA policies and requirements. III.PREVENTIVE MAINTENANCE AND REPAIR: All preventive maintenance and physical repairs will be done by the contractor through a warranty. IV.Authorized Dealer, Distributor or Reseller of Original Equipment and Warranty: Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. V.SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
