- The Department of Veterans Affairs Medical Center, NCO 9 Network Contracting Office, on behalf of Lexington VA Medical Center, is issuing this sources sought synopsis as a means of conducting market research to identify parties having an interest in and the resources to support this requirement for a firm-fixed priced Full Service Contract for St. Jude Velocity and Ensite equipment. The result of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 811219. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals or quotes; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. If your organization has the potential capacity to perform these services, please provide the following information: 1.Organization name, address, point of contact, email address, Web site address, and telephone number. 2.Business size: small, small disadvantaged, woman-owned, Hub Zone, serviced-disabled veteran-owned small business, large business, etc. 3.Business type under North American Industry Classification System (NAICS) 811219, which contains a size standard of $20.5 Million, CAGE Code and DUNS Number. 4.Under which NAICS code does your company usually provide the requirements described in the Statement of Work (SOW)? Please provide rationale for your answer is different from NAICS 811219. 5.Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational and staff capability. The government will evaluate market information to ascertain potential market capacity to: 1.Provide services consistent in scope and scale with those described in this notice and otherwise anticipated. Secure and apply the full range of corporate financial, human capital, and technical resources required to successfully perform similar requirements. 2.Implement a successful plan that includes: compliance with program schedules; cost containment; meeting and tracking performance. 3.Provide services under a firm-fixed price contract. BASED ON THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH, THIS REQUIREMENT MAY BE SET-ASIDE FOR SMALL BUSINESSES OR PROCURED THROUGH FULL AND OPEN COMPETITION. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding their submissions. Submission Instructions: Interested parties who consider themselves qualified to perform the services are invited to submit a response to this Sources Sought Notice by 8:00am CST, June 21, 2016. All responses under this Sources Sought Notice must be emailed to LaShanda.Harrington@va.gov, Subject Line Title: Full Service Contract for St. Jude Velocity and Ensite Equipment. SAM: Interested parties shall be register in the System for Award Management (SAM) as prescribed in FAR Clause 52.232-33. The SAM can be obtained by accessing the internet at www.sam.gov or by calling 1-866-606-8220. Scope of Work: Work shall be performed at the Lexington VAMC located at 1101 Veterans Drive, Lexington, KY 40502. Contractor shall provide a Full Service contract for St. Jude Velocity and Ensite equipment. Services shall include: annual preventive maintenance, all parts, and labor and loaner equipment if required. 1.The preventive maintenance shall be in accordance with the original equipment manufacturer (OEM) latest specifications. PM services shall include, but need not be limited to, the following: cleaning of equipment, reviewing operating system software diagnostics to ensure that the system is operating to the manufacturer's specifications, calibrating and lubricating the equipment, performing remedial maintenance of non-emergent nature, testing and replacing faulty and worn parts and/or parts which are likely to become faulty, fail or become worn, inspecting, and replacing where indicated, electrical wiring and cables for wear and fraying, providing documentation of services performed. 2.Each respondent must have an established business, with an office and full time staff. The staff includes a "fully qualified" FSE and a "fully qualified" FSE who will serve as the backup. 3."Fully Qualified" is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized training program, for the equipment identified. For field experience, the FSE(s) has a minimum for two (2) years' experience, with respect to scheduled and unscheduled preventive and remedial maintenance, on St. Jude's equipment. 4.The FSEs shall be authorized by the contractor to perform the maintenance services. All work shall be performed by "Fully Qualified" competent FSEs. The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model of the equipment covered by this contract. The CO may authenticate the training requirements, request training certificates or credentials from the contractor at any time for any personnel who are servicing or installing any VAMC equipment.
