- COMBINED SYNOPSIS/SOLICIATION VA246-16-Q-0320 ABLATION CLAMPS (I)This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA246-16-Q-0320. (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88. (IV)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (V)The Network Contracting Office 10-SAO CENTRAL intends to award a firm-fixed price service contract for Ablation Clamps and pens for the Cleveland VAMC, 10701 East Boulevard, Cleveland, OH 44106-1702 (VI)The North American Industrial Classification System (NAICS) code for this procurement is 339912 with a business size standard of 1000 employees This procurement is unrestricted to and for all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. (VII)The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price and meets the requirements of the SOW. Quotes are due by 10:00 AM Eastern Standard time on an June 13, 2016 Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Statement of Work A.GENERAL INFORMATION 1. Title of Project: Cardioblate Ablation Devices and Pens 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified The LSCDVA Medical Center has a requirement for additional Cardioblate ablation devices and pens. 3. Background: The LSCDVA Medical Center Surgical Service is looking to procure a Cardioblate ablation devices and pens to replace consumed stock for Cardiac surgical procedures. 4. Performance Period: The period of performance is __One-Time Purchase 5. Type of Contract: Firm-Fixed-Price 6. Place of Performance: LSCDVA Medical Center 7. Performance Based Contract: N/A B. CONTRACT AWARD MEETING We will waive the meeting requirement. C.GENERAL REQUIREMENTS Description Quantity 1Cardioblate Gemini-S Ablation Device6 2Cardioblate Ablation Pen XL3 D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Task One: Ship six Cardioblate Gemini-S Ablation Devices Deliverable One: Six Cardioblate Gemini-S Ablation Devices Task Two: Ship three Cardioblate Ablation Pens Deliverable Two: Three Cardioblate Ablation Pens E. EVALUATED OPTIONAL TASKS AND ASSOCIATED DELIVERABLES [If applicable] None F. SCHEDULE FOR DELIVERABLES 1. The contractor shall complete the Delivery Date column in Attachment A for each deliverable specified. 2. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response in accordance with applicable regulations. G. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. P. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by the VA which have been generated by the contractor in the performance of this task order are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2.The CO will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the CO for response. 3. Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO.
