Description
Wage Rates SF 1449 (fill in form) Statement of Work for USCG AIR STATION OPA LOCKA FL This combined synopsis/solicitation for services is prepared in accordance with the format in subpart 12.6 of the Federal Acquisition Regulation (FAR) as supplemented with additional information included in this notice. This synopsis/solicitation is being advertised as Total Small Business Set-Aside only. This announcement constitutes the only solicitation; no seperate solicitation will be issued. The solicitation number is HSCG28-16-Q-AA456. Applicable North American Industry Classification Standard (NAICS) codes are: 238210 Size Standards of $15.0 This requirement is for a fixed price contract. The contract will be awarded using Simplified Acquisition procedures in accordance with FAR part 13. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation that will be the most advantageeous to the Government, price and other factors considered. The following factors shall be used to evaluate the proposals: 1. Technical Capability of the services offered to meet/exceed the Governments Requirement per the attached statement of work. 2. Vendor past performance 3. Price of items offered to the Government When submitting your proposals, the Government requests a price breakdown of the following: a. cost of labor and supervision for the installation of (1) Fire Alarm Control panel. (1) remote annunciator, (1) smoke detector, approximatley 2300 ft of THHN wire. approximatley 2300 PVC Pipe, (9) monitor modules, (1) cost for underground cable with sod to not be restored and last but not least a total labor cost. All quotes MUST be submitted on the Standard Form 1449, which has been attached. Blocks 17a, 24, 26 (total award amount) with signatures. Anticipated award date: 10 business days after the close of this solicitation, OOA 6/24/16 @ 3:00pm Eastern Standard time. Quotes are to be received using only email to: debra.a.smith@uscg.mil Unit POC: LT Runco 305.953.2326 All contractural questions MUST be sent via email to: debra.a.smith@uscg.mil. Phone calls will not be accepted. Telephone of quotes will NOT be accepted. A formal notice of changes (if applicable) will be issued in FEDBIZOPS ( www.fbo.gov ) Wage determinations: 2005-2119 Rev Date: 12/29/2015 (Attached document) Statement of Work is attached Period of Performance: Performance of work is expected to commence NLT 10 days after award is made. Extension period must be approved by the Contracting Officer for any unforeseen delays The Contractor shall provide all labor, supervision and any other supplies, materials and equipment necessary to perform the services required in the Statement of Work. Copies of the HSAR clauses my be obtained electronically at http://dhs/gov. All quotes submitted MUST have the vendors DUNS # and Federal Tax ID #. If they do not, they will not be evaluated in the final decision. All vendors MUST be registered in SMA (System for Award Mangement) prior to the close of this solicitation: https://www.sam.gov/portal/public/SAM/. The vendors SAM registration must be in the "ACTIVE" status prior to the closing of this solicitation.
