- VA69D-16-R-0904 Acute Stroke Referral Services - Tomah VA Medical Center The Great Lakes Acquisition Center (GLAC), on behalf of the Tomah VA Medical Center (hereinafter called VA), intends to issue a sole source five (5) year contract to Gundersen Lutheran Medical Center, 724 Denton Street, La Crosse, Wisconsin 54601 (hereinafter called Gundersen) pursuant to 38 U.S.C. 2304(c)(1), FAR 6.302-1 Only One Responsible Source and No Other Supplies or Services will Satisfy Agency Requirements. The VA has an immediate need to contract with a Joint Commission (JC), certified Primary Stroke Center that is within a close proximity to the VA. Tomah VA is classified as a Supporting Stroke Facility (SSF) which means that it is a facility with limited capabilities relating to staffing, technical coverage, study interpretation, or appropriate numbers and types of beds that does not allow for consistent care of patients preventing with acute stroke. Gundersen is a JC certified Primary Stroke Center that can meet the treatment needs for VA stroke patients that also has the capability to provide emergency medical air transportation. Gundersen has a helicopter that is available 24 hours a day, 365 days a year that can be airborne within minutes of notification. The air transportation is accredited by the Commission on Accreditation of Medical Transport Systems. Sources interested in responding to this notice are required to submit a capability statement that includes experience and certifications. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purposes of determining whether to conduct a competitive procurement or to proceed with a sole source contract. Questions to this announcement must be submitted in writing to the Contracting Officer no later than 1:00PM Central Time, June 9, 2016 to the following email address: Patrice.Bond@va.gov. No other form of communication will be accepted.
