Description

The Veterans Affairs VISN 12 sites including: CAPT James A. Lovell FHCC, Clement J. Zablocki VA, Edward Hines, Jr. VA, Jesse Brown VA, and William S. Middleton Memorial VA are seeking potential sources capable of providing all compounded sterile preparations for parenteral use. The compounding pharmacy will be contracted to provide the following compounded sterile preparations utilizing FDA-approved sterile components to sites in VISN 12 named: PCA- Fentanyl Hydromorphone Morphine Epidural- Bupivacaine Hydromorphone + Bupivacaine Morphine + Bupivacaine Drip- Epinephrine Norepinephrine Other- Phenylephrine The compounding pharmacy must provide the compounded sterile products in barrel syringes/bags that are compatible with the CareFusion Alaris pump systems; the VISN 12 sites are not to provide compatible barrels. Any shortages or recalls in compounded sterile preparations will be communicated effectively to the sites and support will be provided as applicable. Other compounds may be added to the list or substituted for those already on the list provided they are mutually agreed upon by both the compounding pharmacy and the VISN 12 sites. The compounding pharmacy will be responsible for the sterility, potency and stability of their compounded sterile preparations in order to enhance patient safety and prevent infection. The compounding pharmacy must maintain registration with FDA as an outsourcing facility, must comply with CGMP requirements and with other FDA requirements. Upon FDA inspection in which a Form FDA-483 was issued, the compounding pharmacy must share that information with the VISN 12 pharmacy sites named in the SOW and must provide a plan to correct any deficiencies identified. The compounding pharmacy must provide initially and at least annually responses to the American Society of Health-Systems Pharmacy's 'Outsourcing Sterile Products Preparation Vendor Assessment Tool'. No formal solicitation document exists at this time. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned (SDVOSB); firm's experience in providing the listed service; and any other pertinent information that demonstrates the capability to meet the above requirement.
