- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) - Southern Tier in North Chicago, IL is soliciting businesses for preventive maintenance of Siemens De-Ionized closed loop water systems located at the FHCC in North Chicago, IL. The two systems are currently running in place at their remote locations, and the manufacture has recommendations for filter changes, sanitization procedures, etc., that must be performed at designated frequencies based on the amount of water that is used by the service/end user. Siemens Medica Model R200, s/n MR200E250712 Siemens Centra Model CN200RDM1 s/n 0022052881 The Contractor shall provide all labor, materials, and transportation necessary to complete the required work. The NAICS Code is 811219 with a Size Standard of $20.5 million in revenue. The Great Lakes Acquisition Center is conducting a market survey to determine the existence of potential vendors with the capability to fulfill this acquisition. Contractor must be able to meet all requirements of the Statement of Work. Interested parties shall provide a brief overview of their firm's capabilities and past experience with projects similar in scope to this project. Capability statements shall also include: a.DUNS number b.Organization Name c.Organization Address d.Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e.Business size (small business, other than small business, Woman-Owned, etc...) f.Socio-economic status ( If SDVOSB or VOSB - provide SDVOSB or VOSB CVE verification) (All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments.) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 811219. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than June 17, 2016 at 11:00 AM (CST).
