- Small Rural Hospital Transition Project The Health Resources and Services Administration (HRSA) is issuing the Request for Quotation (RFQ) under the referenced number. Your organization is invited to submit a quote in accordance with the requirements set forth in the attached solicitation. The period of performance is for a base year of 12 months with four (4), 12 month option years, if exercised. This is 100% Small Business Set-Aside. A Firm-Fixed Price type purchase order shall be issued. The NAICS Code for this procurement is 541990 and the PSC Code is R425. Questions or issues for which you require clarification or correction by the Government must be furnished electronically via email to Ms. LaTarsha Rogers at lrogers@hrsa.gov. In order to allow a reply to be provided to all prospective Offerors before submission of their price quote, your questions must be received no later than 9:00 AM Eastern Standard Time (EST) on Tuesday, June 7, 2016. Questions received after the due date and time shall not be considered. NO EXCEPTIONS. Your proposal must be signed by an official authorized to bind your organization and submitted to LaTarsha Rogers by email in PDF format at lrogers@hrsa.gov, no later than Thursday, June 23, 2016 at 9:00 am Eastern Standard Time (EST). You must also request a delivery receipt to ensure timely receipt of proposals. The purpose of this procurement is to provide on-site technical assistance to nine (9) small rural hospitals. The technical assistance services shall prepare nine (9) small rural hospitals for transitioning to the new health care environment. Eligible hospitals must be located in a rural (non-metro) persistent poverty county or a rural census tract of a metro persistent poverty county as defined by FORHP and have 49 staffed beds or less as reported on the hospital's most recently filed Medicare Cost Report.
