Description

Compliance Testing for Clean Rooms, Biological Hoods and Fume Hoods Muskogee, OK VA Medical Center Tulsa, OK VA Outpatient Clinic Network Contracting Office 16 is requesting information only, regarding a company to come to our facility and perform Compliance Testing for Clean Rooms, Biological Hoods and Fume Hoods, located at the VAMC in Muskogee, Oklahoma. See Statement of Work for specifics. Please communicate via email to eric.tuck@va.gov by May 26, 2016, 11:00 AM CST as to your company's ability to perform service per the attached Statement of Work. IMPORTANT NOTE: This is for requesting information only no award will be made off of this RFI. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. The Veterans Administration thanks you for your time and assistance related to this Request for Information. Compliance Testing for Clean Rooms, Biological Hoods and Fume Hoods Specification and Work Statement BRIEF SCOPE OF WORK (This is for information only) SERVICE A.1.The contractor shall certify clean rooms, biological hoods, and fume hoods in accordance with the latest revision of the following standards: a. ISO 14644-1:1999(E)' b.USP 797 c.NSF 49 d.Manufacturer's Specifications A.2.The Contractor shall provide service at the Jack C. Montgomery VA Medical Center, Muskogee, OK to include all labor, travel expenses, tools, test equipment, materials, HEPA filters, and other incidentals necessary to certify the following items: a.Muskogee USP 797 Hoods: 1.Class II Chemo Hood Nuaire Model 425-400 S/N 110220102406 2.Class II Chemo Hood Nuaire Model 425-400 S/N 76707 AFN 3.Class 5 Clean Bench Nuaire Model 301-630 S/N 76726 AFN b.Muskogee USP 797 Clean Rooms: 1.IV Prep Room BB1OOB2-53 2.Ante Room BB100B-53 3.Chemo-Therapy Room BB1OOB1-53 4.HEPA Filter Integrity c.Muskogee Lab hoods: 1.Class II Type A2 Nuaire Model 425-600 S/N 116227071207 2.Class II Type A2 Nuaire Model 425-600 S/N 116211071207 3.Hamilton Fume Hood EE#5410 A.3.The Contractor shall provide service at the Ernest Childers VA Outpatient Clinic, Tulsa, OK to include all labor, travel expenses, tools, test equipment, materials, HEPA filters, and other incidentals necessary to certify the following items: a.TOPC Lab Hoods 1.Class II Type A/83 Labconco Model 36209-08 S/N 265898 2.Kewaunee Fume Hood EE#9752 A.4 The Contractor shall provide certification services for each item every 6 months.It is recommended that all items be certified collectively on a single site visit. The workday for each inspection shall be from 8:00 to 4:30 p.m., Monday through Friday excluding Federal holidays, and scheduled in advance with the COR in coordination with the Chief of Pharmacy and Chief of Laboratory Services.
