Description
Sources Sought The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 8), James A. Haley Veterans Hospital, and Tampa, FL is currently seeking firms i.e. Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hub zone, 8(A), Woman Owned Small Business OR Small Business with the ability to provide the items listed below. The contractor must meet the American Nurses Credentialing Center (MAGNET) accreditation requirements. Businesses responding shall indicate their socio-economic status for example: small business disadvantaged small business, HUBzone, woman owned business, veteran owned small business, service disabled veteran owned small business, etc. The Small Business Administration website has the small business defined: http://pro-net.sba.gov/cgi-bin/closemesba.pl?TO=https%3A%2F%2Feweb1%2Esba%2Egov%2 Fnaics%2F. The NAICS code for this sources sought is 541910- Marketing research and public opinion polling. This has a size standard of $15.0 million. The government reserves the right to consider a SDVOSB, VOSB or small business set-aside based on responses to this source sought. All information submitted by respondents to this sources sought synopsis is voluntary. This sources sought notice is not to be construed as a commitment by the Government, nor will the Government reimburse any costs associated with the submission of information in response to this notice. Respondents will not individually be notified of the results of any Government assessments. The Government's evaluation of the capability documentation received will factor into any possible forthcoming solicitation. Interested individuals shall provide a capability statement, to include complete address, phone & fax number of contact. All emails from interested parties must be received by 05:00 PM, 20 May, 2016. Patient Satisfaction Survey Performance of Work Statement 1. Purpose To establish a national, external patient satisfaction survey contract at James A. Haley VA Hospital (JAHVH). 2. Scope The contractor shall administer patient satisfaction surveys for a sample of inpatients and outpatients discharged from the James A. Haley VA Hospital (JAHVH) and provide internet based access to the survey results. The existing sampling plan is fed by automatic downloads. Data feeds have been established for the following service lines: Ambulatory Surgery; Inpatient (4S, 4N, 5S, 6S, 7N, 5NPCU, CICU, SICU, MICU); Inpatient Rehabilitation (POLY-2 REHAB-3, CPRP-3); Spinal Cord Injury (SCI-B, SCI-D, SCI-DR, SCI-E, SCI-F, SCI-FV); Inpatient Behavioral Health (ARC I, ARC-II); Outpatient Clinics (Alpha Clinic, Bravo Clinic, Charlie Clinic, Delta Clinic, Foxtrot Clinic, Golf Clinic); Emergency Department ; and approximately 150 Long Term Care/Nursing Home (CLC-C, CLC-D, HPCU). Data will be generated for approximately 40 reporting Units and 8 thousand discharged population per week. The number of population discharge varies per units. Results are used for quality improvement efforts in all of these departments to maintain veterans' satisfaction with delivery of care. These reports are also used when applying and qualifying for Baldrige and Carey awards; as well as applying and re-designating for the American Nurses Credentialing Center's Magnet Recognition Award. Service lines/units may be added or deleted as required by the Government during contract performance. The survey results shall be updated on a monthly basis. Weekly, the contractor will be provided via automatic electronic feed with a list of names and addresses of patients recently discharged from the James A. Haley Veterans Hospital (JAHVH) and the long term facilities on a regular schedule basis. Surveys will be conducted by mailing or e-mailing patient satisfaction surveys to an appropriate sample of these patients. Surveys for patients on the long term/nursing home care/spinal cord units will be sent to James A. Haley Veterans Hospital (JAHVH) for hand delivery to the patient, family member or appropriate patient designee. These questionnaires are a result of nationally accepted criteria using Eight Dimensions of Patient-Centered Care and behavioral-based questions which may include standard and/or Hospital consumer Assessment of Healthcare Providers and Systems (HCAHPS). The survey results will be made available to the James A. Haley Veterans Hospital (JAHVH) through a secure internet based solution. The survey must be maintained externally to the VA hospital system. 3. Requirements The contractor shall provide statistically valid and reliable survey results of the patient customer experience. The contractor shall meet the following requirements: Administer satisfaction surveys by e-mail, mail or hand delivered to the patient or family member to a sample of patients discharged from or currently living in a long term or spinal cord unit at the (JAHVH). The contractor must mail or make available for hand delivery surveys for a sample of patients which is considered adequate by generally accepted statistical principles. The minimal statistical confidence range must be 95% or higher as indicated in the quality assurance plan. Develop a wide range of patient survey questions criteria in collaboration with the Nursing Quality Improvement team at James A. Haley Veterans Hospital (JAHVH). The criteria will include but are not limited to, staff courtesy, pain management, privacy, communication with staff, availability of physicians, availability of nurses, ability of staff to address anxieties/fears, wait times, satisfaction with educational materials, explanation of test results, and overall satisfaction with care. The Contractor shall allow modifications and/or additions of survey questions at the request of the James A. Haley Veterans Hospital (JAHVH). The survey results data shall be compiled on a secure internet site which will be accessible to members of the James A. Haley Veterans Hospital (JAHVH). This survey data shall be organized in a fashion which allows the James A. Haley Veterans Hospital (JAHVH) to analyze responses and response rates from approximately 40 nursing unit in the facility as indicated in paragraph 2. All survey data shall be archived for the life of the contract on a none-VA secure internet site in order to allow the staff at James A. Haley Veterans Hospital (JAHVH) to analyze trends. The contractor shall use Psychometric validation survey and analysis methodologies to deliver valid and reliable results based upon standard industry accepted statistical principals. The contractor shall maintain the security and confidentiality of all data obtained from the contract relationship with the James A. Haley Veterans Hospital (JAHVH). The data shall be organized at the unit/department/organization levels as indicated in paragraph 2 to allow for performance improvement efforts at each level. The performance levels are indicated by each unit/department/organization levels at the James A. Haley Hospital (JAHVH). Prior to being sent out to the patients each surveys shall indicate the unit/ service line /department/organization as refereed in paragraph 2 for which the survey is being conducted. The contractor shall understand and comply with all information safeguarding procedures applicable to the Health Insurance Portability and Accountability Act (HIPAA). The contractor shall never use, release, reproduce, redistribute, or publish any data obtained for any other purpose except for those expressed in this contract without a written permission from the Contracting Officer. Contractor shall possess 3 Years of health care and direct experience in providing patient satisfaction surveys to healthcare institutions of similar size and complexity with high volume. The experience must be external from surveys used by Government agencies (not internal surveys collected and/or analyzed by the Government) and must be nationally benchmarked and recognized (such as by ANCC Magnet). Contractor shall continuous provide leading industry specific process (benchmark)by which the James A. Haley Veterans Hospital will be capable of comparing and updating its own process every 3-6 months. Contractor shall submit with technical quote. Contractor shall provide benchmarks with other Magnet-designated facilities in accordance with ANCC Magnet Requirements. The James A. Haley Veterans Hospital (JAHVH) may change benchmark comparison groups as the needs of the organization change. Contractor shall submit with technical quote. 4. Technical support The contractor shall provide technical support to the James A. Haley Veterans Hospital (JAHVH) through 5 on-site visits during the contract period which will be coordinated with the COR, videoconferencing as needed and 3 meetings and/or teleconferencing as needed. The meetings will include one initial kick-off meeting immediately after contract award, one annual performance meeting per year, and one additional meeting per year if needed at the convenience of the James A. Haley Veterans Hospital (JAHVH). The Vendor will be provided with the Contracting Representative (COR) points of contact at kick off meeting for any coordination in regard to the contract. 5. Period of performance The contract shall be for a period of performance of one base year and four option years (five total years). Option years will only be exercised based on satisfactory performance established in the quality Assurance plan (QASP), continuing need and availability of funds. 6. Location of performance While the majority of work will be completed via the internet and teleconferencing, occasionally on-site support at the James A. Haley Veterans Hospital JAHVH may be required as indicated in paragraph 4. The James A. Haley Veterans Hospital (JAHVH) is located at 13000 Bruce B Downs Blvd, Tampa, FL, 33612. 7. Invoicing Invoicing shall be quarterly, in arrears. 8. IT Security Requirements Files shall be submitted through a secure data exchange. This is not an SFTP/FTP, but it is a secure format that many clients utilize to submit files. The secure data exchange site uses SSL with 128 bit strong encryption to secure data during transport. The utility is as easy to use as sending an email and creates a receipt of the transaction that is mailed back to the sender of their records. 9. Type of contract This contract will be Firm-Fixed-Price (FFP). In-scope changes may be required to accommodate additional work/services and will be accomplished through bi-lateral modifications. 10. No Sub-Contracting The contractor shall not subcontract any work performed under this contract and will avoid any third party involvement regarding data collection, distribution, analysis, and evaluation. All work will be performed solely by the contractor. Minimal consulting is allowed to assist the offeror as long as they only consult. The consultant cannot provide separate services that the offeror is responsible for providing (to include data collection, distribution, analysis, evaluation, etc.). 11. Quality Assurance Surveillance Plan The Government will use the Quality Assurance Surveillance Plan (QASP) attached to this solicitation to monitor contractor performance. 12. Business Associate Agreement The contractor must agree to comply with and sign the Business Associate Agreement (BAA) attached to this solicitation prior to award of the contract. A current national BAA with the VA will be acceptable. A copy should be submitted with your offer.
