- Statement of Work Upgrade Egress Doors Bld C 2nd Floor OR Suite 1. Contract Title. Upgrade to Egress Doors on Second Story ER Suite, Project # 508-16-2-537-0044. 2. Background. This project will upgrade all egress doors in the ER Suite. Many doors are locked, with the explanation that the fire alarm releases the lock. This is not recognized by the Code. The doors must be operable in the direction of egress at all times. This is necessary for patient and staff safety. This project will also provide Von Duprin Panic devices on doors in the direction of egress. These currently have magnetic locks that are operated by an electric key pad. This change is necessary for patient and staff safety. 3. Scope and Deliverables. The Vendor shall provide all material and labor to upgrade all egress doors (5 sets of Double Doors 46"W X 83"H) in the ER Suite. This work involves removing existing non labeled doors (frames remaining in place) and replacing with fires rated doors and hinges, and adding Von Duprin parts (Door loops, Power supplies ( to control Horton brand automatic door closers), push pull kits, UL listed armour plates, brush sweeps, Electrified fire rated panic devices, 18 ga. T astragals, low profile glass kits with clear tempered 1/4'' glass and fire bolts). The project shall be conducted in accordance with VA specifications, design guides, design manuals, design alerts, and all applicable codes and regulations. The contractor shall prepare a complete package to include cost estimates, specifications, and drawings for construction solicitation. 4. Specifications. N/A 5. Performance Monitoring 1.)The Owner's Contracting Officer's Representative (COR) shall meet with the Contractor following the Notice to Proceed and prior to commencing any work. 2.)The COR shall conduct weekly project review meetings with the Contractor. 3.)The COR shall make daily inspections of the worksite. 4.)The COR shall review and approve each phase of installation prior to Contractor moving to the next phase. 5.)The COR shall provide a final inspection upon project completion. 6.)The Contractor shall furnish as-built documentation upon project completion. 7.)See Project Specifications for additional requirements. 6. Security Requirements 1.)The contractor/subcontractor employees will have to sign the Contractor Rules of Behavior (Appendix D) on an annual basis. The contractor/subcontractor will be required to complete the required security and privacy awareness training initially and then annually thereafter, if the contract has not been terminated or completed. 2.)The contractor shall provide to the contracting officer and/or the COR a copy of the training certificates and certification of signing the Contractor Rules of Behavior for each applicable employee within 1 week of the initiation of the contract and annually thereafter, as required. 3.)It is the responsibility of the contractor/subcontractor to immediately notify the COR and simultaneously, the designated ISO and Privacy Officer for the contract of any known or suspected security/privacy incidents. 4.)See Project Specifications for additional Security Requirements. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A 8. Basis of Award. Contractors providing equal products shall provide descriptive literature and shall specifically address, by number, any salient characteristic their product fails to meet. ***THERE WILL BE A SITE VISIT 9AM at the Atlanta VAMC. Award will be made on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards. Include a milestone plan addressing the install process and period. Please provide pricing one a separate pdf. 9. Risk Control The VA will conduct an Infection Control / Risk Assessment (ICRA) as described in Medical Center Memorandum No. 138-11. This contract will be a Class C II project that is limited to a small area of the building exterior. All infection control, occupational health and safety, security and life safety measures identified in Memorandum 138-11 will be implemented throughout the project. See Project Specifications for additional project and Contractor risk control requirements. 10. Place/Period of Performance. This work will take place at the Atlanta VA Medical Center, 1670 Clairmont Road, Decatur, GA 30033.
