Description

THIS IS A SOURCES SOUGHT NOTICE. This is not a request for quote. A solicitation will not be issued at this time. The Department of Veterans Affairs, VA Caribbean Healthcare Systems (VACHS) -Puerto Rico has a requirement for Home Oxygen Services. The intent of the procurement is to award a Requirements Contract. The performance period will consist of one base period and four (4) separate option years. Scope of Requirement: There were 300 prescriptions referrals for Home Oxygen Services/Durable Medical Equipment (DME) set-up made by the VA Caribbean Healthcare Systems, Puerto Rico during the past fiscal year (01 October 2014 thru 30 September 2015). The Performance Work Statement (PWS) which will be published at a letter posting with the solicitation defines the effort required for the setup, installation and management of Home Oxygen Services/DME Set-up, in accordance with Joint Commission (JC) standards, for any Patient treated by the VACHS -Puerto Rico (Vieques and Culebra), Outpatient Clinics (OCs) and all Community Based Outpatient Clinics (CBOCs) supported therein. VACHS, Puerto Rico does not have the capacity to provide all necessary services so a contract will be sought for the provision of home oxygen/ durable medical equipment and all associated products and services. The Contractor shall manage Government-Furnished Equipment (hereafter GFE) and rental oxygen equipment by providing storage, delivery and set up, maintenance and repair services of high-pressure cylinders, oxygen concentrators, ventilators, compressors, oxygen refill systems, emergency backup systems and supplies to eligible VA beneficiaries as required by the VACHS. All services shall be performed in the veteran's place of residence or in a nursing home housing the veteran. The area of service shall be all patients serviced by VA Caribbean Healthcare Systems, Puerto Rico (Vieques and Culebra), including catchment areas (St. Thomas, St. John and St. Croix). Contractors shall have a facility in Puerto Rico and resources or subcontractor in St.Thomas, St. John and St.Croix from which they shall provide service. The vendors shall not deliver the high-pressure cylinders by airplane. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number and DUNS number; (2) if you represent firm as Veteran; proof of registration in the VA Vet Biz and (3) capabilities statement. Award cannot be made to a firm not registered into the System for Award Management (SAM) database. Registration may be accomplished at www.sam.gov and http://www.vip.vetbiz.gov, respectively. The response date to this Sources Sought notice is May 20, 2016 at 11:00AM ET time. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, full & open competition or limited sources procurement is appropriate.
