- schedule statement of work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as a Request for Quotation (RFQ), # IHS-16-011. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87-2 effective 04-06-2016. The associated North American Industry Classification System code is 541511. This requirement is a 100% SMALL BUSINESS SET-ASIDE. Line Items include a base year plus 4 one-year option periods and 1 line item for implementation. See attached Schedule. See attached Statement of Work (SOW) and flow chart. Dates and place of delivery and acceptance: IHS National Supply Service Center, 501 NE 122 Street, Suite F, Oklahoma City, OK 73114. FAR Provision 52.212-1, Instructions to Offerors-Commercial Items, applies and is incorporated by reference with full force and effect as if included in full text. FAR Provision 52.212-2, Evaluation -Commercial Items applies. Evaluation criteria. FAR Provision 52.212-3, Offeror Representation and Certifications-Commercial Items: offerors shall include a completed copy of the provision with their quote. FAR 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition and is incorporated by reference with full force and effect as if included in full text. FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition and is attached with applicable clauses. 52.217-5 Evaluation of Options 52.217-8 Option to Extend Services FAR 52.217-9, Option to Extend the Term of the Contract FAR 52.227-14, Rights in Data-General Additional clauses are attached. Additional Information: The Indian Health Service (IHS) National Supply Service Center (NSSC) intends to award a firm fixed-price simplified acquisition threshold contract in response to the RFQ. The performance period is one year from date of award plus 4 one-year option periods. This requirement is a 100% small business set-aside. Evaluation Factors: 1. Experience with federal government supply inventory/ordering management processes. 2. Timeframe for implementing an inventory management system at a fully functional level. 3. Timeframe for training customers and NSSC personnel on using the inventory management system. 4. Past performance. 5. Provision of requested quote documents. 6. Price. Price is secondary to the technical / non-price factors stated above. The provisions and clauses for this requirement may be accessed electronically at https://www.acquisition.gov/. 1. The Government will award a contract, resulting from this solicitation, to the responsible quoter/offeror whose quote offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. QUOTE SUBMISSION INSTRUCTIONS: All responsible small business quoters/offerors may submit a quote offer, which shall be considered by the Agency. Quotes offers with requested services/product and information shall be submitted to the IHS National Supply Service Center, Attn: Sara Marcus, 501 NE 122 Street, Suite F, Oklahoma City OK 73114 or by email to sara.marcus@ihs.gov with cc to amber.mcquerry@ihs.gov no later than 4:00 p.m. Central Standard Time, May 31, 2016. The quote must be submitted with RFQ number visible and include, quoter/offeror name and all other required information. To be considered for award, the following must be submitted. Failure to provide any of the documents below will eliminate the quote. a. Completed QUOTE SCHEDULE (attached). b. Document or letter summarizing the quote information. Limited to 3 pages. c. Document stating the delivery timeframe (number of days) and tasks required to install/implement the supply inventory ordering management system. Include system description/functions/capabilities and system/IT requirements for IHS, i.e. server specifications, tasks required. Delivery timeframe and tasks document shall be limited to 20 pages. d. Training description and delivery timeframe for NSSC staff and NSSC customers. Web training for NSSC customers shall be acceptable. Live, instructor led on-site training for NSSC staff is required. Training shall be completed before NSSC and NSSC customers start using the supply inventory ordering management system. Delivery timeframe for training and training description shall be limited to 5 pages. e. A list of three (3) previous or current customers/contracts similar in size and scope. Period of performance must be within the last 3 years. Include business name, point of contact telephone number and email, contract description, and total contract amount. (1 page per reference) f. Offeror Representation and Certifications-Commercial Items: offerors shall include a completed copy of the provision with their quote. RFQ questions shall be submitted by email to sara.marcus@ihs.gov with copy to amber.mcquerry@ihs.gov no later than May 23, 2016, 4:00 p.m. CST. RFQ questions will be answered by amendment to the RFQ. Telephone questions will not be accepted. Questions received after May 23, 2016, 4:00 p.m. CST will not be addressed.
