Sources Sought Announcement Posting Date: May 10, 2016 Type: Sources Sought Agency/Office: Department of Veterans Affairs, Network 10 Contracting Office Title: Nursing Home Health Care NAICS Code: 623110 - Nursing Care Facilities (Skilled Nursing Facilities) PSC: Q402 - Medical - Nursing Home Care Contracts Response Date: June 9, 2016, 1:30 pm Description: This is a Sources Sought Announcement. This is NOT a solicitation for proposal, proposal abstracts or quotations. The purpose of the Request for Information (RFI) is to obtain knowledge and information for acquisition planning purposes. The Department of Veterans Affairs Network 10 Contracting Office is conducting market research to identify potential sources that meet the requirements generally described in this notice. The intent of this announcement is to: -Notify potential sources about the requirement -Identify interested vendors The Department of Veterans Affairs, Network 10 Office, on behalf of Battle Creek, VA Medical Center 5500 Armstrong Road, Battle Creek, MI 49037 is seeking to acquire services for nursing home residential care. The Government wishes to have a plan for a base year plus (4) option years for a total of five (5) years of service. Information Requested: Availability of nursing homes whose' s care meets the health needs and promotes the maximum well-being of VA patients within Battle Creek MI catchment area. Battle Creek is looking for nursing home care in or near the following zip codes: oBattle Creek, Michigan: 49012, 49017, 49037, 49015, 49014 oKalamazoo and Portage area: 49024, 49002, 49001, 49008, 49006, 49004, 49048 To include, in or near the following areas: oGrand Rapids area: 49053, 49504, 49505, 49506, 49507 oLansing area: 48906, 48917, 48911, 48864, 48823 Nursing home care shall be furnished to ensure the total medical, nursing, and psycho-social needs of VA beneficiaries. Physician visits, laboratory, x ray, and other special services for VA patients will be at the same frequency as that provided to other patients at the nursing home receiving the same or comparable level of care. In addition, the care provided will include room, meals, nursing care, and other services or supplies commensurate with the VA authorized level of care, without extra charge. The per diem rate(s) established in this contract will include the cost of medical care, drugs, laboratory, x ray, and other special services authorized by VA, unless otherwise specifically accepted. The per diem rate is established by the current Medicaid Rug IV Rates for Medicaid approved nursing homes plus a fair market amount (percentage) to cover the cost of Executive Order 13658, supplies, services, and equipment above that provided under Medicaid established by the local state Medicaid agency (SMA). Rates established after the effective date of this contract will constitute a modification to the contract. VA will use Medicaid rates for room, board, and routine nursing care, RUG IV Services: -Reduced Physical, Behavior & Cognitive -Performance -Clinically Complex -Special Care Low -Special Care High -Extensive Services -Rehabilitation -Rehabilitation + Extensive Services -Ventilator-Dependent -Special Bed Dependent VA Contracted Nursing Homes must Pass 4 out of 7 Criteria listed on the Medicare.gov/nursing home compare website to be eligible for continued VA Contract Nursing Home program. These criteria are as listed: 1. Deficiencies: (Facility fails if there are 3 levels G or worse deficiencies in the current survey 2. Health Requirements Deficiencies: (The total number of health requirements deficiencies cannot be more than twice the State Average in the current survey) 3. Staff Treatment of Residents and Facility Licensure: (Fail with Rating E-L or higher. OSCAR Level E-L equals Nursing Home Compare of 2 or higher) 4. RN Hours: Fail if less than the State Average 5. Total Nursing Hours: Fail if less than State Average 6. Nursing Deficiencies: (Includes Nursing (F353), Nursing Aid Training (F494), Nursing Aid Competency (F495), Nursing Aid Registry Verification (F496), Regular In-service Training (F497), Proficiency of Nursing Aids (F498), Staff Qualifications (F499) 7. Quality Measures: A facility fails when 6 or more of the CMS Quality Measures listed in Nursing Home Compare fall above the state Average. *All Immediate Jeopardy tags (IJ tags) will be reported to the COR/CNH Coordinator within 24 hours of receipt from the State. Nursing Home shall remain registered in the SAM database after the initial registration, the Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the SAM database to ensure it is current, accurate and complete. Vendors interested in this RFI may provide a response that includes information on your firm's capability to provide nursing home care. Please also provide your firm's DUNS number, organization name, address, business size and type as validated via the System for Award Management (SAM) (e.g., 8(a), WOSB, VOSB, SDVOB, or HUBZone small business) pursuant to the applicable NAICS code. Respondent's points of contact, including names, titles, addresses, telephone and fax numbers, and e-mail addresses. Instructions for Submittal Written responses shall be provided electronically, by 1:30 p.m. EST on June 9, 2016. The subject line of the e-mail message shall read: "Battle Creek, VA Medical Center Nursing Home Health Care RFI" and shall be addressed to the attention of: shelly.julien@va.gov The VA will not accept telephone, facsimile, courier, mail, or hand delivery methods of response.
