- SF-1449 (RFQ) FAR Clauses Wage Determination SOW.Attachment.1 Statement of Work (SOW) Synopsis: This combined synopsis/solicitation for commercial supply and service is prepared in accordance with the format in subpart 12.6 of the Federal Acquisition Regulation (FAR) as supplemented with additional information included in this notice. This synopsis/solicitation is being advertised for Small Business Set-Aside vendors only. This announcement constitutes only the solicitation; no separate written solicitation will be issued. The solicitation number is HSCG37-16-Q-PQQ155. This requirement is for a Fixed Price Service Contract. The contract will be awarded using simplified acquisition procedures in accordance with FAR part 13 and FAR part 15. Award determination will be made using Lowest Price/Technically Acceptable. Applicable North American Industry Classification Standard (NAICS) code: 562910: Remediation Services (Asbestos Removal Contractors) *Vendors must ensure that the above listed NAICS codes are listed on their SAM registration to perform this type of service. When submitting your proposal: Proposals may contain one line item with an overall extended price, however it must be itemized showing cost break down. For example, Line One: Labor, Line Two: Materials, Line Three: Shipping. Target Range: $9,000.00 to $14,000.00 Anticipated award date: Approximately 5 business days after close of solicitation, Friday, June 03, 2016. Proposals are to be received no later than close of business 1500 (3 p.m.), Pacific Standard Time (PST), on Tuesday, May 31, 2016. Request vendor complete the following blocks on the Solicitation for Commercial Items (SF-1449): Blocks 17a, 23, 24, 30a, 30b, 30c, and return to address listed in block #9, marked for Attn: SKC Leanna Evans. Mailed proposals sent via traceable means and post marked prior to RFQ close date/time will be accepted to the address listed in block # 9, Attn: SKC Leanna Evans, of the Solicitation for Commercial Items (SF-1449), Faxed proposals will be accepted via fax at 707-839-6136 or via E-mail to Leanna.J.Evans@uscg.mil. Proposals submitted to either this posted mailing address, fax number, or email address will be considered for the award. Proposals submitted to other than the listed mailing address, fax number, or email address will be considered non-responsive. Proposals sent via traceable means (i.e. U.S. Postal Service, Fed-Ex, UPS, etc.) must be received at the below address before RFQ Closing Date. Performance Period: Contractor shall complete this project within 10 days after award has been made to the Contactor. Site Visit: It is recommended contractors attend the site visit being held at USCG Sector Humboldt Bay, 1001 Lycoming Way, McKinleyville, CA 95519, in the Chief's Mess on Tuesday, May 24, 2016 at 0900 (9am) Pacific Standard Time (PST). FAR 52.236-2 Differing Site Conditions FAR 52.236-3 Site Investigations and Conditions Affecting the Work Work/delivery hours: Perform all on-site work between the hours of 7:00 AM and 5:00 PM, Monday through Friday (Federal Holidays excluded), unless otherwise approved by the Contracting Officer and the Unit Commanding Officer. No work will be performed on weekends unless pre-arranged with the Contracting Officer's Technical Representative (COTR) or Contracting Officer.
