THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS NOT A REQUEST FOR QUOTES OR PROPOSALS. THE PURPOSE OF THIS NOTICE IS FOR MARKET RESEARCH PURPOSES ONLY AND A SOLICITATION IS NOT CURRENTLY AVAILABLE, AND THIS NOTICE DOES NOT INFER THAT ANY SOLICIATION WILL BE MADE BY THE GOVERNMENT IN THE FUTURE. Network Contracting Office 1, located in VISN 1, is seeking information for market research purposes only. Response to this notice and the information requested will assist the government in determining the appropriate acquisition method. A solicitation is not currently available and the government may or may not issue a solicitation based on the information collected. A response to this notice is considered by the government to be voluntary, and any response to this notice is at no cost to the government. The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 1) has issued this Sources Sought Notice for the purpose of identifying parties that are capable and interested in providing a combination of a one-time supply and installation of new audiology equipment. This also includes services to the done for the relocation and installation of existing equipment located on station at the Edith Nourse Rogers Memorial Veterans Hospital/ Bedford VAMC located in Bedford MA, VISN 1. In conducting market research for qualified parties, the government is interested in identifying vendors able to provide both this supply and service, both from a GSA/FSS schedules contract holder or from other small businesses with the capability from non-holders GSA contract holders. (Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hubzone, 8(A) OR Small Business) Applicable NAICS Code is 334510, Audiological equipment, electro medical, manufacturing with a Small Business size standard of 500 Employees. Firms desiring consideration shall email a capabilities statement and the following information directly to the Contracting Specialist referencing the following in the subject field, " VA241-16-N 0402- Install Audiology Booths." The booths should meet the form/function and fit as outlined in the attached Scope of Work. This includes technical specifications of listed equipment, and the size requirements of equipment needed at the Bedford VAMC. Place of Performance: Edith Nourse Rogers Memorial Veterans Hospital/ Bedford VAMC, 200 Springs Rd, Bedford, MA 01730. Response: Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the Scope of Work attached. Responses should include the following information: Business name, address, business size and Socioeconomic Category (HUBZone, 8(a), Service Disabled Veteran, Small Business, Large Business, etc.), and point of contact to include e-mail address and telephone number. All information is to be submitted via e-mail to the Contract Specialist. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 4:00 pm (EST) on Friday, May 20, 2016. No responses will be accepted after this date and time. Questions can be submitted electronically to Ronald.Kline@va.gov. Install Audiology Booths Model RE-243 Double Wall Audiometric Test Booth and Model 140 ACT-4 RF Shielded Audiometric Testing Suite The VA Bedford Medical Center requires providing and installation of 1 ETS-Lindgren Model RE-243 Double Wall Audiology Booth and installation of 2 IAC Audiology Booths in the new Audiology Clinic, Building 2, 1st floor. SCOPE OF WORK 1.Contractor shall provide and install 1 ETS-Lindgren Audiology Booth. Booth information is as follows: a.Model RE-243 Double Wall Audiometric Test Booth i.Shop drawing and information pamphlet is available. 2.Contractor shall complete the installation of the 2 IAC Audiology Booths. Booth information is as follows: a.Model 140 ACT-4 RF Shielded Audiometric Testing Suite i.Shop drawing and information pamphlet available. ii.Both booths require modification to the door and floor of the control room side of the booth to match the floor level in the Audiology Clinic. The new door dimensions and floor level shall match the testing room side of the booth. 3. APPLICABLE DIRECTIVES a. Install Booths in accordance with install instructions for Model 140 ACT-4 RF Shielded Audiometric Testing Suite and Model RE-243 Double Wall Audiometric Test Booth. b. Design and installation shall meet latest edition of the NEC, NFPA, VA Master Specification Section 01 74 19, and all applicable building codes. c. All work shall be performed in accordance with OSHA codes. 4. PERFORMANCE REQUIREMENTS The following tasks are required: a. Meet with VA Engineering staff and perform field work necessary to determine required materials and equipment required. b. Schedule arrival date at least 72 hours in advance with VA Engineering. Installation will need to be coordinated with construction contractor. c. Request through Contracting Officer to use any government equipment as necessary. d. Move panels (12 crates) to site. VA Engineering will not provide a forklift and driver. Crates are currently at the Bedford VA Bldg. 19 approximately ½ mile from the install site. e. Install Booths in accordance with install instructions. f. Provide VA Engineering with a list of any broken or missing parts. g. The site shall be left clean of all debris and hazardous materials. Contractor is responsible for removal of any waste unless prior approved for on-site disposal by VA Engineering. Installing contractor shall use all reasonable means to divert demolition waste from landfills and incinerators, and facilitate their salvage and recycle in accordance with VA Master Specification Section 01 74 19. h. Contractor shall schedule punchlist-style walk through and repair or otherwise fix any issues applicable to the install of the booth. i. All work shall be completed within 90 days after receipt of order. This is a Sources Sought Notice and responses will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ) or Request for Proposals (RFP), nor is the government obligated to issue an RFQ or RFP. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey.
