- ENG FORM 4025R Excavation permit Solicitation The Defense Logistics Agency, Defense Supply Center has a requirement for Concrete Repair located at the Defense Supply Center Columbus, Ohio. This procurement is issued as a 100% small business set aside. The NAICS for this requirement is 237310. The Small Business size standard for this requirement is $36,500,000.00. The award will be a firm fixed contract. Award will be based on the lowest evaluated price of the technically acceptable proposal meeting all of the solicitation requirements from responsible offerors with acceptable past performance. There will be one award made for this project. Award may be made without discussions. The following summary is for the purpose of general information and is not intended to include or describe every feature or item or to define the scope of work. It is expected that prospective contractors will attend the site visit, review solicitation documents, and thoroughly familiarize themselves with the project prior to submitting a proposal. One preproposal conference/site visit is scheduled for this project and is scheduled for 10:00 AM local time on 19 May 2016. All individuals wishing to attend will be required to request Center access no later than 24 hours prior to the site visit date. Those individuals wishing to attend must provide their name and corporate affiliation to barbara.bocsy@dla.mil by email. The Defense Supply Center is a secure facility and individuals who do not preregister will not be granted access. Registered participants can not bring unregistered personnel on center with them. Individuals will need to enter the Center through the Visitor Processing Center at the North Yearling Road Gate and valid federal or state issued photo identification will be required. PROJECT INFORMATION: The Request for Proposals will solicit for a single award firm fixed priced contract for work to be performed at the Defense Supply Center Columbus located in Columbus, OH. SCOPE: This project consists of the following: replacing concrete sidewalks, curbs & slabs; constructing new sidewalks; sealing cracks in concrete pavement; re-sealing joints in concrete pavement; and performing partial and full depth repairs in concrete pavement The Contractor shall furnish all labor, material, supplies, equipment, tools, delivery, disposal and supervision required in performing all operations in conjunction with this project complete, in strict accordance with these specifications and Contract Drawing No. 5-422 subject to the terms and provisions of this contract. CONTRACTOR QUALIFICATIONS: This project requires the use of specialized contractors capabilities and experienced staffing. The general contractor and all key sub-contractors shall have a minimum three (3) years' experience in all facets of construction projects under the assigned NAICS code. Offerors must provide information regarding their Technical Construction Capabilities and Technical Qualifications of Proposed Staffing, to include the individuals who will be responsible for Project Management, Quality Control, and Safety Compliance. A) Past Performance: Offeror shall provide past procurement contracts and references for projects satisfactorily completed within the last three (3) years of similar size and scope. A minimum of 3and a maximum of 5 past contracts. B) Technical Construction Capability: A written technical construction management plan shall be submitted by the offeror to demonstrate their understanding and capabilities of completing all work in accordance with the Statement of Work and project drawings. The plan must identify all work that can be accomplished with in-house resources and demonstrate that they are capable of obtaining any project requirements that cannot be self-performed. The technical construction management plan must identify how the offeror will directly superintend the work, perform project and quality control management, comply with safety and environmental regulations; any project requirements that cannot be self-performed must be identified (e.g. subcontractors list and manufacturers of key material components for the project) The offeror shall provide this information with their company proposal submission. C) Technical Qualifications of Proposed Staffing: Written qualification statement for the individual who will be responsible for project management of the project. Qualifications The Project Manager must have a minimum of 5 years experience as a Project Manager or Superintendent on projects of similar size and complexity. Written qualifications statement for the individual who will be responsible for quality control in accordance with General Requirements Section 01 45 0010 20 Paragraph 1.6.1. Qualifications An individual with a minimum of 5 years combined experience as a superintendent, inspector, QC Manager, project manager, or construction manager on similar size and type construction contracts which included the major trades that are part of this contract. The individual must be familiar with the requirements of the EM 385-1-1 and have experience in the areas of hazard identification and safety compliance. Construction Quality Management Training In addition to the above experience and education requirements, the QC Manager shall have completed the course Construction Quality Management for Contractors and will have a current certificate. Written qualifications statement for the individual who will be responsible for safety in accordance with FAR 52.236-13 Accident Prevention (Nov 1991). Accident Prevention includes compliance with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR Part 1910; and compliance with all pertinent provisions of the latest version of U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, in effect on the date of the solicitation. This individual will be designated as the Site Safety and Health Officer (SSHO). Qualifications and Training The SSHO(s), as a minimum, must have completed the 30-hour OSHA Construction safety class or as an equivalent, 30 hours of formal construction safety and health training covering the subjects of the OSHA 30-hour course (see Appendix A, paragraph 4.b) applicable to the work to be performed and given by qualified instructors. > The SSHO is also required to have five (5) years of construction industry safety experience or three (3) years if he possesses a Certified Safety Professional (CSP) or safety and health degree. The written qualifications statement shall be certified and signed by a principle of the offeror. Offerors shall submit a resume of each key individual. This will be evaluated to determine if they have the appropriate technical training, certifications, and previous construction experience to meet the technical requirements of a project of this nature. If any of the submitted individuals will perform more than one of the listed functions, this is acceptable as long as the submitted documentation identifies any and all functions the given individual will be performing. Details of workmanship shall be in accordance with the highest standards and best practices of the respective trade as recognized by respective contractor's association. Only qualified tradesmen or mechanics shall be assigned to do the work required by this contract. At no time shall the work be accomplished by apprentices or laborers without the supervision of a journeyman or higher level supervision. Contractor personnel will not require a secret clearance. The contract will be required by law to pay prevailing Service Contract Act wage rates for all work awarded under the contract. The Defense Supply Center Columbus is located in Franklin County, Ohio. PERIOD OF PERFORMANCE: All work associated with the performance of this project must be completed within 120 calendar days after issuance of the Notice to Proceed. Questions concerning this requirement must be submitted no later than 4:00 PM local time on 31 May 2016. The solicitation will close at 1:00 PM (Eastern) on 09 June, 2016.
