Description
NOTICE TO CONTRACTOR This is a COMBINED SOLICITATION/SYNOPSIS for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to negotiate a Firm Fixed Price (FFP) contract, utilizing procedures under FAR part 13, for Commercial Items as described in the schedule below. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The Request for Quotation (RFQ) number N00259-16-T-0124 is issued as a Request for Quotation (RFQ). The closing date is 12 May 2015 at 06:30 AM Pacific Standard Time. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-87. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The North American Industry Classification System (NAICS) Code for this acquisition is 81130; Size: 750. This opportunity is for full and open competition. Naval Medical Center San Diego requests responses from qualified sources capable of providing the following services and a firm-fixed price contract shall be awarded. The Naval Medical Center San Diego is seeking the following tendon allografts. The part numbers are Precision Allograft Solution, but anything equal will be reviewed. The contractor shall provide the following: CLIN 0001 - Semitendinosus tendon: Unit of Issue: each, Quantity: 10 Unit Price: _______ Total CLIN Price: _____ CLIN 0002 - Tibialis tendon anterior: Unit of Issue: each, Quantity: 5 Unit Price: _______ Total CLIN Price: _____ CLIN 0003 - Gracilis tendon: Unit of Issue: each, Quantity: 5 Unit Price: _______ Total CLIN Price: _____ The products must: 1. Have non irradiated cadaver tissue. 2. Be processed using PASCO technology to ensure sterility and strength 3. The size must be at least 8mm diameter x 25mm length. 4. Most importantly, the product must be semitendinosus, anterior tibialis and gracilis tendon. The Government will only consider firm fixed price quotations.
