- Title: Data Analysis NOTICE OF INTENT TO ISSUE A CONTRACT. The Centers for Disease Control and Prevention, National Institute for Occupational Safety and Health (NIOSH), Division of DSHEFS, 4676 Columbia Parkway, Cincinnati, OH 45226, hereby announces its intent to issue a contract to Ohio Bureau of Worker's Compensation, 13430 Yarmouth Drive, Pickerington, OH 43147 to develop worker compensation data outcomes in the state of Ohio. Task 1: Develop Database of Linked ODJFS and OBWC Data The OBWC will develop a database of Ohio Department of Job and Family Services (ODJFS) employment data by quarter [e.g. number of employees by Ohio establishments in calendar year 2016] identified according to North American Industry Classification System (NAICS) codes linked with the OBWC workers compensation (WC) loss data. The OBWC will provide NIOSH with a documentation file of steps taken to establish the database. Task 2: Develop Workers' Compensation Data Tables Using the above database, the OBWC will develop workers compensation (WC) data tables detailed in Appendices 1-2 from calendar year 2016 for all NIOSH industry sectors. All data tables will provide data for the full range of NAICS industry code specificity available (e.g. 1 to 6 digit level), all establishment sizes combined, and all OBWC-insured establishments (with Single and Multiple Location Private employers and Single and Multiple Location Public employers data clearly distinguished). Data do not have to be delivered according to the NORA classifications. OBWC can choose to deliver the data according to the industry structure most convenient for their processing (e.g. 2 digit NAICS etc.). The OBWC will provide NIOSH the requested data tables listed under Task 3: Develop Workplace Wellness Grant Program Data Tables The OBWC will develop Workplace Wellness Grant Program (WGP) data tables as detailed in Appendix 3. The OBWC will provide NIOSH the requested data tables listed under VII. DELIVERABLES. Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. Please forward the capability statement and pricing information to Dwight Favors, Contract Specialist, Reference RFP 2016-Q-65073, Centers for Disease Control, NIOSH, ATTN: Dwight Favors, MS: C-4, 1090 Tusculum Avenue, Cincinnati, OH 45226, dyf3@cdc.gov. All vendors must be registered in the System for Award Management (SAM) prior to an award of a federal contract. The website is: www.sam.gov The Government will review any/all capabilities statements and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 5.102, a solicitation will be issued to Ohio Bureau of Worker's Compensation, 13430 Yarmouth Drive, Pickerington, OH 43147, as the only source and a contract will be issued without any additional notices being posted.
