- The Department of Veterans Affairs, Network Contracting Office (NCO) 19 has a requirement to provide Health Care for Homeless Veterans (HCHV) to eligible Veterans and is seeking providers that have experience and a proven record of successfully working with beneficiaries recovering from co-morbid conditions such as serious mental illnesses, substance abuse disorders and other serious medical conditions. The VA anticipates multiple indefinite-definite/indefinite-quantity (IDIQ) contract awards to satisfy this requirement. Services are sought for existing facilities within the catchment areas of the following VA Medical Centers: Eastern Colorado Health Care System, Denver, Colorado; Grand Junction, Colorado; and Fort Harrison, Montana. The initial performance period shall be a base period of one year, with options for 4 one-year periods, in accordance with clauses in the solicitation and awarded contracts, to extend the term of the contract, at the discretion of the Government. The resultant contract is designed to serve homeless Veterans in need of immediate placement in a safe environment with on-site treatment services through the Health Care for Homeless Veterans (HCHV) Program. The goal of the HCHV Contract Residential Care Program is to rapidly stabilize Veterans' medical, mental illnesses, substance abuse disorders and other psychosocial problems in order to place Veterans in other appropriate transitional or permanent housing within 60-90 days, but no more than 180 days. The VA intends to engage a provider to provide rapid placement of VA-referred Veterans in a safe, residential treatment setting while addressing Veterans' complex needs through on-site clinical treatment and case management services. Services shall consist of room and board, including at least three (3) nutritionally adequate meals a day, seven (7) days a week; laundry facilities for residents to do their own laundry or to have laundry done; therapeutic and rehabilitative services determined to be needed by the individual patient in a plan, developed by the offer with input from the patient and the VA health care for homeless veterans case manager. The Offeror shall be required to address the following evaluation criteria and must be submitted as part of their proposal: 1) Mission Capability; 2) Past Performance; 3) Cost/Price. Multiple contracts may be awarded with or without discussion, to the responsible offeror(s) whose proposal conforms to the evaluation factors listed in FAR 52.212-2, "Evaluation - Commercial Items", taking price and non-price factors into consideration. Interested Offerors will be responsible for downloading copies of the solicitation (and any ensuing amendments) from the website without further notification from the Contracting Officer. No hard copies will be made available. The solicitation will provide all necessary information relating to the proposal submission process. Offerors are encouraged to register their interest in the solicitation on the FBO website, thereby ensuring notification of the issuance of the solicitation and subsequent amendments. The North American Industry Classification System (NAICS) code is 624221, Temporary Shelters, with an SBA size standard of $11.0 million. All other terms, conditions and other requirements will be included in Solicitation VA 259-16-R-0023, which will be posted on or about May 25, 2016, on the Federal Business Opportunities website, https://www.fbo.gov. Offerors shall be due on or about June 8, 2016. Solicitation Number VA259-16-R-0298 must be referenced on all correspondence regarding this Pre-solicitation Announcement and Solicitation. Interested firms are reminded that in accordance with FAR Part 4.11, prospective Offerors are required to have an active registration in the System for Award Management (SAM) (formerly CCR Registration and ORCA Certification) Database and FAR Part 4.12, the submission and maintenance Representations and Certifications also in the SAM database.
