**** Begin Word Document - 'FBO-DESCRIPTION' ***** This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Items, and is being conducted under FAR Subpart 13.5- Simplified Procedures for Certain Commercial Items apply. The solicitation number is VA256-16-R-0317 and is issued as a Request for Proposal (RFP) The proposed contract action is for home health care services for which the Government intends to solicit competitive offers. Interested persons may submit proposals, and all eligible proposals received prior to the response date listed on FBO will be considered by the Government. There is a small business set aside. The NAICS code is 621610 and the size standard is $15 Million. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83). Only emailed requests received directly from the requester are acceptable. Servicing Facility is Veterans Healthcare Center of the Ozarks, 1100 North College Avenue, Fayetteville, AR 72703. RESPONSES ARE DUE WEDNESDAY, MAY 11, 2016, by 3:00 p.m. Eastern Standard Time (EST). Proposals are due electronically to sheronda.stewart@va.gov no later than the response date listed on FBO.
